Nurse Practitioner - Remote | WFH

We are seeking a skilled and dedicated Nurse Practitioner to join our healthcare team. This position is based in an office setting and requires expertise in Remote Patient Monitoring.

Responsibilities

Conduct thorough medical assessments

Manage a primary care facility for Medicare and Medicaid patients

Diagnose and treat acute and chronic illnesses

Develop and implement personalized patient care plans

Provide health coaching and education to patients and their families

Collaborate effectively with healthcare professionals to coordinate patient care

Utilize electronic medical records system for accurate documentation and coding

Requirements

Valid Nurse Practitioner license in [state]

Master's degree in Nursing

Minimum of 2 years of experience as a Nurse Practitioner

Own NPI for billing purposes

Excellent communication and interpersonal skills

Ability to work collaboratively within a multidisciplinary healthcare team
