This week Cadence passed a significant milestone in that we now serve 🌟 25,000 🌟 patients across the country. Even more noteworthy is that 60% of these patients are located in rural or underserved areas. Here’s a few reasons why: 🌉 Bridging the gap: Access to quality healthcare remains a challenge in rural and underserved areas. By extending care into these communities, Cadence is making strides toward equitable healthcare access for all. 🏥 Improving health outcomes: Patients in underserved regions often face higher health risks due to limited access to medical resources. Our high-quality remote patient care program ensures they receive timely care, which is crucial for managing chronic conditions and preventing hospitalizations. 💪 Empowering patients: By providing easy-to-use cell enabled devices coupled with our 24-7 clinical Care Team, we empower patients to take charge of their health, regardless of their zip code. This autonomy leads to better health management, more independence, and improved quality of life. 👩⚕️ Supporting healthcare providers: Rural healthcare providers often face significant workload pressures. Our solutions support them by providing continuous patient data, enabling more informed and proactive care. Thank you to our dedicated team, partners, and the patients who trust us. Together, we are transforming healthcare and making a real difference where it’s needed most.

We are incredibly proud that our work on Patient Reported Outcome Measures (PROMs) has been selected as a finalist in the Healthcare Outcomes category at this year’s LaingBuisson Awards. Our commitment to improving patient health outcomes is at the heart of everything we do. Through our expanded PROMs initiative we are collecting more patient feedback than ever before, which in turn is helping us guide higher quality care and patient experience. Here’s how we're leading the way: 💡 Partnering with My Clinical Outcomes Ltd to make it easier to reach more patients and gather more feedback through digital solutions. 💬 Thanks to our digital innovation, we’re achieving unprecedented engagement rates with our patients meaning we are getting more valuable feedback than ever before. 📈 Exceeding the standard requirements on PROMs reporting, with 70% of our surgical patients now eligible for PROM questionnaires. We’re incredibly proud of our Work on PROM’s and it's a bonus to be shortlisted in these awards. Stay tuned for the award ceremony in November, where we HCA UK teams have been shortlisted in other categories too! #HCAUK #HCAHealthcareUK #LaingBuissonAwards #HealthcareInnovation #PatientOutcomes #DigitalHealthcare #PROMs #LBAwards

Interesting article, discussing the Dartmouth General Hospital’s recently launched pilot project aimed to enhance patient monitoring in the ER. Patients experiencing critical symptoms are now given wireless pulse oximeters, allowing nurses to continuously track vital signs like pulse and oxygen levels, even while patients wait. As the article mentions, this innovative approach has already proven invaluable, enabling healthcare providers to detect and respond to abnormalities that might otherwise go unnoticed. Lori Sanderson, Health Services Director for DHG, highlights the project’s goal to boost patient safety during extended wait times by leveraging real-time data. When following proper protocols, piloting innovative health technologies is crucial in driving healthcare forward. It allows for real-world benefits, ensures the feasibility of broader implementation, and directly contributes to stronger health outcomes. As outlined in the Shared pan-Canadian Interoperability Roadmap, harnessing innovative technologies and standardizing data is essential in creating a health system where data can flow seamlessly and drive efficiencies across the entire health system. As the roadmap and the Working Together Bilateral Agreements are in the implementation stage, I'm looking forward to staying updated on innovative healthcare pilots, such as this, and how they can contribute to a reimagined and fully interoperable health-system. https://lnkd.in/gR4csxyk

EMPOWERING DIGITALLY ENABLED CARE WITH THE CPT® CODE SET START DATE:4/25/2024 START TIME:12:00 PM CDT / 10 AM PST Current Procedural Terminology (CPT®) codes are a uniform language that allows health care professionals to communicate the work they perform to streamline reporting, and to help increase accuracy and efficiency. By reflecting the very latest in medical care provided to patients, the CPT codes enable physicians and patients to benefit from innovative care models and technology. There are so many questions providers have when implementing new digital modalities (telehealth, remote physiological monitoring, eConsults). The AMA is offering a free webinar that is a must watch. This webinar will explore real-world examples of digitally enabled care in action. These case studies were released earlier this year as part of the AMA’s Future of Health series. The case studies highlight successful collaborations between health systems or physician practices and digital health companies to facilitate digitally enabled care and drive the future of medicine for the betterment of public health. On Thursday, April 25, join the AMA and leaders from Omada Health and Oshi Health to learn more about their approach to care delivery, how they collaborate with health systems and practices to drive outcomes, and the importance of leveraging the CPT code set to generate evidence for these new models of care.
