Interesting Spectator article arguing the military have become “obsessed with safety”. Referencing Sydney Decker, it argues the army has “safety by bureaucracy” Some highlights for those unable to access the article: ▶️ The military’s approach to safety is grounded in Lord Justice Haddon-Cave’s report on the 2006 Nimrod accident, which cost 14 lives. ▶️ Haddon-Cave laid down four principles of military safety: ‘strong leadership from the very top’, ‘independence throughout the regulatory regime’, ‘much greater focus on People in the delivery of high standards… not just on Process and Paper’ and keeping ‘regulatory structures, processes and rules as simple as possible so that everyone can understand them.’ ▶️ Service personnel are 56 per cent less likely to die each year than their civilian peers. ▶️ The combination of more rules and less training over the past decade has led, many in the Army feel, to a loss of confidence, especially among vehicle operators, making them less competent and safe. ▶️ Defence has also shifted the focus away from having clear lines of responsibility. A designated staff officer in each brigade headquarters can, and regularly does, override commanding officers by forbidding the use of certain vehicles, based on centrally-held online records of the safety checks on those vehicles. ▶️ Training for war requires an approach which teaches people to assess, perform and take risks when tired, cold and frightened – circumstances where accidents are inevitably more likely. When a mishap happens, you can be sure our United States Air Force Safety experts are on it, investigating the what to determine the why. But with that comes what's called PSI, or Privileged Safety Information. Safety privilege ensures that analyses, conclusions and recommendations are shared only on a need-to-know basis, for DoD mishap prevention. This cannot be used for public release, disciplinary action, or any other non-mishap prevention purpose. In some cases, investigators may offer a promise of confidentiality to witnesses to help give safety investigators a rapid and unimpeded path to the truth. The ultimate goal--to prevent future mishaps and ensure combat and mission readiness. #safety #mishap #readiness #USAF

I don’t think this will hit the mark. A system can be lied to; moreover, things like this are put in place to signal “hey, I’m doing something about this” while not actually getting to the core issues. I’ve seen this in many forms over my time as an aircraft maintainer. The pure intent is there, but this isn’t the approach. What mishap prevention takes is the leadership saying that they will tolerate delays for the sake of safety. Do that, and be deliberate about ensuring the production team is looking for those issues—the guy that didn’t get enough sleep, etc. This initiative is great for data points, provided it doesn’t get populated with lies because they’re trying to keep production up. You have to set the conditions for the right decision to be made—we have to give them decision space. After that, you can address things person-to-person to find out how you can help them. #aircraftmaintenance #culture

When emergency response decision makers encounter chemical based emergencies, they depend on sound information resources as a guide. As we bridge into 2024, a reminder to all that, the new updated version of the emergency response guidebook will hit the street sometime in the first qaurter.
