We had this issue when nurse led walk in centres were set up over 20 years ago. criticisms of nurses supplying medicines on PGDs. now it's business as usual. The Clinical Pharmacy programme there were criticisms of the work to embed pharmacists in General.practice, yet the forward thinking practices saw this as organisation change worth investing in. At NHSE was was significant transformational support for GP federations and Practices. Both examples above was to provide more capacity in primary care. The conversation with General practice is what support do you need from Community Pharmacy? What services could community pharmacy manage for you ? Both Community Pharmacy and General Practice are under pressure and the focus should be on collaboration, education and putting the patient first. Pharmacy first has just started and all participating Community Pharmacies should be praised. The way Community Pharmacy is being shaped needs the same transformational support. Community pharmacy stood up during Covid and the sector is standing up now.

Encouraging update from Kaufman Hall in the just-released February 2024 #Hospital Flash Report -- margins for hospitals (in general) are positive, especially buoyed by improving labor expenses. While prescription drugs and supply expenses are up, January 2024 financials for U.S. hospitals look healthier relative to the past couple of years. See the bar chart for the up-tick. Kaufman Hall also reports that there may be a shift toward value-based payments which could be reflected in hospitals' net revenue not rising as fast a gross revenue -- a data point we'll keep watching to see if #ValueBasedCare is growing in 2024. cc Erik Swanson thx for the update! How is Pharmacy First performing so far? We're three weeks in. 🎯 Before launch 63% of pharmacists said they have had no training - GPs are pointing out they're seeing high numbers of patients being referred back to general practice via Pharmacy First 🎯 We know nearly 400 pharmacies closed last year and that only 4% of pharmacists felt Pharmacy First could be delivered within their current workload - at my (excellent) local pharmacy there are really long waits for just a prescription following closures in my area 🎯 Polling would indicate patients support the scheme but in terms of execution the public health message is surely hard to land when we're talking seven specific conditions, all with their own T&Cs attached (we look at earache but only if you're between 1 and 17, UTIs only women 16-64...) and especially hard if even government ministers don't understand the small print Is the staffing and funding in place for this to work? Will it really save up to 10m GP appointments a year? Big thanks to Dr. Zhang Hao Tian from 1 Medical Group for taking the time to educate our group of self-employed individuals on Lasting Power of Attorney (LPA) and Advanced Medical Directives (AMD). His insightful talk at our ERA office was beneficial and much appreciated by Hexa Group. LPAs and AMDs are crucial legal documents that help individuals maintain control over their affairs, especially during times when they may not be able to make decisions for themselves due to illness or incapacity. LPAs allow individuals to appoint trusted individuals to make important decisions on their behalf, such as managing finances or making healthcare choices, if they become unable to do so themselves. AMDs, on the other hand, outline a person's preferences for medical treatment and end-of-life care. It ensures that their wishes regarding medical interventions, life support, and organ donation are known and respected. Having these documents in place provides peace of mind for both individuals and their loved ones, ensuring that their wishes are honored and their affairs are managed according to their preferences, even in challenging circumstances.
