Follow-up to my earlier post about the great MINS meeting in Breckenridge earlier this month. One common theme throughout the meeting was that "minimally invasive" is a concept, not a definition. There are many different ways of doing certain procedures, whether that be a fusion or decompression (like a lumbar diskectomy). Using the example of the surgical treatment of the patient who required removal of a symptomatic lumbar disk herniation, what I took home from the meeting (ie, IMHO, your mileage may vary) as well as the many other meetings I've been to over the last 30+ years was that it mattered LESS which technique (microsurgical, microsurgical tubular approach, endoscopic uniportal, endoscopic biportal) one used as long as you were doing the right procedure at depth. In otherwords, how you achieved this was less important than the mastery with which you achieved the decompression and removal of the offending fragment(s). That mastery can only come from a combination of mentorship, training, practice, and a thougthful committment to excellence on every case.
