Let's continue our analysis.. 😎 We can categorize patients into two groups: those who are bothered by something and don't know what to do about it, and patients who already know how to alleviate their pain and are seeking targeted medical services. In this scenario, we'll establish two primary branches within the TA division: Pain and Service. Depending on the clinic's range of services, these audiences may overlap, as the pain symptoms in a specific location could necessitate one, two, or even three types of services. Intriguing, isn't it? And it gets even more fascinating when we delve into a more detailed analysis: 💁♀️ Ex. Patient K. is experiencing chest pain. Here are the options for assisting him: - General Practitioner (GP) consultation and examination - Chest X-ray - Electrocardiogram (ECG) - Echocardiogram (ECHOCG) - 24-Hour Blood Pressure Monitoring - CT scan of the thoracic spine - MRI of the thoracic spine - Ultrasound of soft tissues - Laboratory analyses - Consultation with specialized specialists - Procedures or surgery if necessary And so on.. 💁♀️ Patient A. is well aware of the necessity of an MRI of the thoracic spine due to concerns about chest pain. (Here, it is not crucial whether this examination is medically necessary; if he is willing to pay for it, we consider the matter resolved for him.) It appears that he also falls under the category of Patient K. However, in this case, the patient is seeking a specific service. It is essential to provide him with information that is as accessible and detailed as possible, covering the entire process from entering the clinic to receiving the results of the examination and outlining any potential subsequent actions. Follow-up to my earlier post about the great MINS meeting in Breckenridge earlier this month. One common theme throughout the meeting was that "minimally invasive" is a concept, not a definition. There are many different ways of doing certain procedures, whether that be a fusion or decompression (like a lumbar diskectomy). Using the example of the surgical treatment of the patient who required removal of a symptomatic lumbar disk herniation, what I took home from the meeting (ie, IMHO, your mileage may vary) as well as the many other meetings I've been to over the last 30+ years was that it mattered LESS which technique (microsurgical, microsurgical tubular approach, endoscopic uniportal, endoscopic biportal) one used as long as you were doing the right procedure at depth. In otherwords, how you achieved this was less important than the mastery with which you achieved the decompression and removal of the offending fragment(s). That mastery can only come from a combination of mentorship, training, practice, and a thougthful committment to excellence on every case. I will discuss this and other aspects of care in my upcoming webinar, "Contemporary Treatment of Lumbar Disk Herniations: a Surgeon's Perspective".
