Dismissal of symptoms, hysteria, Speculums, gravity (to delay preterm labor), Sitz bath, watchful waiting, no clinical guidance, maternity care deserts, no known causes, Lupron, warm compresses, manual tracking of contractions… It has been rewarding to contribute to research, knowledge, innovation, advocacy and be invited speak publicly about #womenshealth (including #maternalhealth) over the past years. Causes, diagnosis, prevention and management of health conditions that only, disproportionately, or differently affect girls, females, and women are still relatively under-researched, underfunded and under-invested in. Many existing treatments and interventions are in need of revision, simplification, being made more accessible and comfortable. It is encouraging that Women’s Health is garnering some deserved attention, a few over the past few months: ✅ the World Economic Forum x McKinsey report on the ~$1Trillion economic value of closing women’s health gap ✅ the White House Initiative on Women’s Health Research and recent $100M announcement of investment ✅ the amazing work by Liz Powell et al to organize founder/CEO, innovators, researchers, interest groups, individuals etc. to provide valuable insights to guide the former ✅ ARPA-H Sprint for Women’s Health ✅Women’s Health having ample ‘stage’ presence at JPM ‘24, VIVE ‘24 etc. ✅ More Personal Storytelling on various platforms ✅ Growing #femtech industry And so many more. This progress is not without challenges from various determinants of health, policy and investment lags, hence the persistence of many women’s health gaps. All stakeholders, men and women, need to ensure women are appropriately served by the health system. ❓What gaps have you observed (in annual PCP or gynecologist visits) that could help women be more proactive about their health? ❓What are other health gaps that if you had a magic wand, you’d love to narrow for the girls and women in your life? This week in #womenhistorymonth, I’ll be joining Kerstin Recker Alexandre, Dana Gilland and Rashmi R. Rao as we explore various aspects of Women’s Health. Bilikis Jumoke Oladimeji MD, MMCi, CPHIMS: I have two personal focus areas (there are so many!). I'm still flabbergasted (as a survivor of preeclampsia) that the medical community does not understand the cause and uses such manual and "watch and wait" approaches to such a prevalent and dangerous medical condition in pregnancy. I suspect this gap is due to limited research opportunities - there is often under diagnosis, researching pregnant women is hard, and if the woman and child survive, the condition may resolve itself shortly after. The other gap I see is similar - vertigo. I've suffered from it for years and there is no explanation and no reliable treatment. During my last ER visit (because it was so bad I wanted to be sure I wasn't having a stroke), the doctor was dismissive, ran minimal tests, and the nurse even told me that there were 3 other women in the ER with virtigo presently - so it must be something to do with the lunar cycle (apparently that is the trend she identified). I would love to see more research and care focus in these areas!

A couple more…(this is fun!) Lack of menopause content/curriculum in med school/rotations. Average time spent is approx. 4 hours. A click down level of detail on female hormones and impact on overall quality of life (mental and physical health). A lot of times we are told an issue or symptom is “hormone related” but not told which hormone, acceptable ranges, expected impact/improvements to symptoms. And there’s gotta be a tie in there of being expected to be everything to everyone at every hour of every day (working mothers in a dual career household, I see you!) and the impact of chronically high cortisol levels and overall mental/physical health. Eve Rodsky has some fascinating perspective here on chronic stress and auto-immune diseases in women. Bilikis Jumoke Oladimeji MD, MMCi, CPHIMS you are a star! Shine on and cannot wait to see what insights you bring to the world.

Reflecting on my journey through the complexities of healthcare, especially within the realm of women's health, is both a harrowing and inspiring experience. The dismissals, the lack of clinical guidance, and the sheer perseverance required to be 'heard' deeply resonate with my own story and those of countless others navigating similar paths. My experiences have highlighted the systemic gaps and biases that unfortunately still pervade our healthcare systems. Yet, it's through these challenging experiences that the most powerful advocacy is born. I literally had to instruct ER doctors to take proper history, that without that they would have clearly missed a life-threatening situation. If I had a magic wand, I would narrow the gap in mental health support for women, especially around reproductive and maternal health. The psychological impact of fertility issues, pregnancy, childbirth, and postpartum challenges often goes under-addressed. Enhancing mental health resources, support networks, and integrating mental health care into women's health services would provide a more holistic approach to women's health and well-being. Thank you for the thought provoking posts Bilikis Jumoke Oladimeji MD, MMCi, CPHIMS . One area of gap in women's health I will like to mention is the soci-behavioural insights to menstruation and menopause. I habe a 15 year old and I am in my 40s. So I can attest to seeing some of the social nuances during menstrual expectations, and for pre-menopause. It seems like there are some undiscussed conversations of the many fears women of different ages habour and the effects on their self esteem, stress level and productivity. I hope there can be socio behavioural research from an informal circle perspective which explores what ladies share as fears in social groups.

I have so many opinions here! I'll start with two and save a comment for each. #1 I wish PCPs and gynecologists understood the impact of peri menopause and menopause on the body that lead to increased glycogen stores, higher cholesterol and weigh gain for starters! None of the generic exercise more (more than half Ironmans!?)
