How did BlueCross BlueShield of South Carolina incorporate social risk to help build personalized care plans? While BCBSSC had existing pop health and care management programs, they identified an opportunity for a more data-driven approach. April Richardson, VP of Clinical Services at BCBSSC, described how they developed a 7-level care management system, ranging from prevention to acute care. While this care continuum was clinically robust, the ways that people interact with and navigate the healthcare system are influenced by so much more than their diagnosis. Overlaying social risk data onto this care continuum equipped BCBSSC to create personalized care plans for their members. #SDoH #SocialRisk

Thanks to Congress's investment in Medicare's advanced alternative payment models, organizations like NW Momentum Health Partners (NWMHP) in Washington State are able to ensure patients' needs are looked after once they are discharged from the hospital or long-term care setting. One patient, Michael has Multiple Sclerosis and needed support following an inpatient stay related to a fall. His caseworker discovered that Michael was living in unsafe housing that led to a greater risk for falls. In collaboration with NWMHP, Michael's caseworker helped him find a clean and safe space to live with caregivers who support his health and wellbeing. #ValueBasedCare models benefit #Medicare patients in many ways, from closely coordinating care delivered by health care providers to improving social circumstances that shape patients' health and wellbeing. You need a solution for waitlist management by March 2024. The 2023/4 NHS Standard Contract states that providers are required to implement an early screening, risk assessment and health optimisation system by March 2024.
