“The Future Directions in Menopause Research: Optimizing Midlife Health of Women Roundtable” is the launch event for NIH’s Women’s Health Roundtable Series on important women’s health topics as part of the White House Women’s Health Research Initiative. This series was developed as a recommended action in response to the Presidential Memorandum to bring attention to priority topics within the Department of Health and Human Services and disseminate information on federally supported research areas. The roundtable series will be led by NIH institute, center, and office leadership and will engage the research community and the public to bring greater awareness to female-specific health conditions (e.g., menopause and endometriosis), as well as diseases and conditions that predominantly affect women (e.g., autoimmune diseases), and present or progress differently in women (e.g., heart disease, Alzheimer’s disease, etc.).

Did you know cardiovascular risk can rise after women go through menopause? According to new findings presented at the American College of Cardiology's Annual Scientific Session women’s cardiovascular risk rises sharply after they go through menopause quickly catching up to men of similar age and health profile. This study indicated that after menopause, women have much less estrogen and shift to a more testosterone-heavy profile,". "This affects the way our body stores fat, where it stores fat and the way it processes fat; it even affects the way our blood clots. And all of those changes increase our risk for developing heart disease. It is important to encourage post menopausal women to visit their doctors, talk about heart disease risk factors, and follow up on recommended tests. Yesterday I had to wear a heart monitor because of risk related to menopause and being a CEO. I’m post menopausal and cardio vascular risks increase significantly. And the cardiac risks for post-menopausal women in leadership roles rockets. In 2023, World Menopause Day highlighted cardiac health as a key risk for and it really hits for women in leadership, boardroom executives, and female founders. Why is this important? Coronary heart disease kills twice as many women in the UK as breast cancer. Stress is a silent killer, significantly impacting cardiac health. The demands of leadership, rapid growth, and investor expectations can lead to immense pressure and stress. Additionally, the risk of cardiac disease and chronic conditions increases significantly in post-menopausal women. So, what can employers especially those leading the wellbeing and leadership development strategy do to help prevent these issues and provide effective solutions? This week, The Lancet, a major medical journal, published an opinion piece that advocates against "over-medicalizing" menopause. Condemnation from menopause advocates, like myself, has been swift and sharp. Here is my full response. https://lnkd.in/gxjTTCDZ Menopause is a hormone deficiency state. Why do we focus so much menopause care on treating symptoms? Yes, we want to treat symptoms. Women should not suffer. The symptoms of menopause are awful. But. But! But, women without symptoms need menopausal healthcare as well. There are plenty of chronic health conditions that the medical community screens for and treats, whether or not the patient has debilitating symptoms. Why is menopause so different? For every woman who is a candidate for hormone replacement therapy, we should be recommending HRT as preventative medicine against heart disease, diabetes, osteoporosis, sarcopenia, genitourinary conditions, dementia, and cancer. ALL women should be offered HRT, not just those with horrible hot flashes.  To my medical community, we must do better. Because women deserve better. 51% of the population is female and will undergo menopause. With more than half of the population expecting this phase of their life, it is essential to have effective treatment options in place, including hormone therapy (HT). Beliefs about HT’s potential side effects and risks have reduced its popularity and use over the last two decades - in large part due to the Women’s Health Initiative (WHI) study that showed a potentially higher risk of breast cancer with the use of combined HT (conjugated equine estrogen (CEE) and medroxyprogesterone acetate) and no improvement quality of life. Since its publication, many reexaminations of the original WHI study have shown that HT may not have the same risks as originally thought. For example, more recent reports from the WHI show that HT is the most effective treatment for the management of vasomotor symptoms and also, when using CEE alone, the risk of breast cancer is reduced by 23%. However, stigma and concerns around HT remain. According to Carrot Fertility’s recent menopause in the workplace report, a third of women in the US and UK reported feeling concerned about potential health risks associated with HT. In addition, many physicians trained in the era of the WHI study, continue to hesitate to prescribe HT to their patients in fear of the risks potentially associated with it. A recent study published in The Journal of The Menopause Society re-examined the Women’s Health Initiative report and aims to dispel many of these misconceptions. HT can be an extremely effective treatment for those who need it and I’m hopeful that this study will be able to clear the air around the use of HT for perimenopausal and menopausal symptoms. I am seeing more & more women in my LinkedIn feed speak out about their experience with periods, menopause, PCOS, endometriosis... And I am LIVING for it 👏 We've got one hell of a long way to go to end medical misogyny, medical gaslighting and the deranged stigma that women's pain = hysteria. Sharing our collective experiences online won't provide a magic solution, but it's a great place to start. (And yes: LinkedIn IS absolutely the right platform to be having these conversations) I'm currently listening to Rebel Bodies by Sarah Graham which highlights some startling - yet sadly not shocking - stats around women's health, including: 🚩 Research shows that 50% of women experience period pain bad enough to need medication. 🚩 Yet the medications to treat period pain, namely the pill and non-steroidal anti-inflammatory medication like ibuprofen, have a 20-25% failure rate. 🚩 A study from 2016 showed that 5x more studies are conducted into Erectile Dysfunction (ED) compared to Premenstrual Syndrome (PMS), despite the former affecting just 19% of men while the latter affects 90% of women. 🚩 An earlier review found that 40% of women with PMS do not respond to any current treatments available. Meanwhile, men can readily buy medication over the counter for ED. And if you want to look at this in monetary terms... 🚩 Endometriosis alone costs the UK economy a whopping £8.2 BILLION per year in treatment, loss of work and healthcare costs. So yes, I will continue to bang the drum and speak out about women's health. And no, I won't be sorry if the words 'uterus' or 'period' offend anyone. I live to see the day when I don't have to have regular surgery to keep on top of my currently incurable disease ✌️ [Picture of me leaving hospital after having 5 hours of surgery. The more women physicians I talk to, the more I realize how important learning about menopause can be... for themselves and their patients. All women (50% of the population) will be living 40-50% of their lives in their post-menopause years. And yet, hardly any medical research includes these women. Every organ system in a woman's body is affected by menopause. So when we experience new onset anxiety or rage, the inability to remember names, find words, or multitask like we used to- this is the brain. When we lose our libido, experience dryness and pain with intercourse, leak urine for no reason, get bladder infections- this is our genitourinary system and this affects our relationships with our partners and with ourselves. (Yet, ED medications are a dime-a-dozen... and no one is telling men it could kill them, but I digress) When we see our mothers get Alzheimer's or break a hip, do we want this to be our destiny? Heart disease is the number one killer of women and the rates skyrocket after menopause. Yet the uninformed fear-mongers tell us- "Don't take hormones because they cause breast cancer!" If you are a woman, and especially if you are a woman physician, do not spread menopause disinformation. Seek out a menopause expert. Send your patients to a menopause expert. Ok, stepping off the soapbox now. #menopausePSA

🎙️NEW PODCAST EPISODE: Ep 17: Hormone Replacement Therapy as Treatment During Menopause with Roksana Karim, PhD, MBBS. 🔍 Karim is an epidemiologist researching women’s health, particularly the impact of menopause and sex hormone concentrations on atherosclerosis/cardiovascular disease and other age-related chronic inflammatory outcomes including bone density and cognition. 🫀 There is an increased risk of atherosclerosis and cardiovascular disease in women right after menopause. This finding inspired Karim to further research changes during menopause particularly estradiol levels, metabolic markers, and atherosclerosis changes in women. 💊Hormone replacement therapy (HRT) replaces estrogen that the body stops producing after menopause. 📉Karim’s research showed that higher levels of estradiol were beneficial in reducing the progression were beneficial in reducing the progression of atherosclerosis.
