Interventional EUS has revolutionized the managment of manay many Gastrointestinal and Mediastinal Diseases. Happy to share one such case of post whipple's surgery peripancreatic walled off necrosis complicated with sepsis with shock. Alhamdulillah , Patient was managed successfully endoscopically with EUS Guided Cystogastrostomy and Direct Endoscopic Necrosectomy(DEN). For more than 6 months, now patient has been doing well.(Alhamdulillah). EUS guided Cystogastrostomy is now the standard of care for symptomatic pseudocysts and walled off necrosis.

Thanks to all our team members at the Department of Critical Care, the First Affiliated Hospital of Sun Yat-sen University. Our paper highlights the potential long-term harmful effects of red blood cell transfusion among ICU sepsis survivors, calling for a more restrictive transfusion strategy. This study evaluates the FUWAI-SAVE system, a modified low-priming cardiopulmonary bypass (CPB) system, in reducing perioperative blood transfusions in adult patients undergoing cardiac surgery with intermediate transfusion risk. In the latest addition to our clinical case series, we illustrate how specimen PET-CT imaging can be used to ensure that a representative biopsy is obtained in the diagnostic work-up of lymphoproliferative disease. 🙌 18F-FDG PET CT is used to stage lymphoproliferative disease, but final diagnosis relies on histopathological confirmation. To obtain this confirmation, a representative biopsy needs to be obtained from an 18F-FDG-avid lymph node. Depending on the location of the lymph node to be biopsied, it can be a challenge to obtain a representative sample. If postoperative analysis shows that the sample is not representative, the patient may need to undergo a second surgery to establish the diagnosis – with all associated costs and worries. ❌ 👉 Specimen PET-CT imaging can provide the surgeon with reassurance that the most relevant lymph node for histopathological evaluation is indeed resected, within minutes at the point of surgery. One of the best things of #interventionalradiology is multidisciplinary collaboration with all kinds of medical specialties. For example, in the Haaglanden Medisch Centrum (HMC) we closely work together with our vascular surgeons to treat patients with vascular diseases 🏥 . Recently, we worked together on this case of a mycotic aneurysm. This +70 years old patient was treated for a pneumococcal sepsis with antibiotics, but clinical improvement stagnated. FDG-PET scan showed focal FDG-uptake and irregular soft tissue around the aorta ☢. A CT scan in the portovenous phase showed subtle signs of aortic wall destruction 🆘 ⏰. In the next 12 days, two CT-scans showed progressive dilatation of the aorta. The decision was made not to wait for the rupture and place a stent in the infected aorta. With my vascular surgery colleagues Willem-Jan de Jong and Christinne Corion we placed a Medtronic Endurant limb prosthesis (ETLW161682) in the aorta, with technically good result 👌. Prior research has indicated that elevated plasma levels of cell-free hemoglobin following extended cardiopulmonary bypass (CPB) in cardiac surgery patients are linked to acute kidney injury (AKI). A recent pilot study has proposed that employing hemoadsorption with Jafron HA380 during CPB is pivotal in lowering cell-free hemoglobin levels, sustaining haptoglobin levels, and diminishing the occurrence of AKI within the investigated cohort.
