To improve health outcomes for seniors after hospital discharge, Rebecca Trotta and colleagues have been reimagining hospital-to-home care transitions with a model called SOAR (Supporting Older Adults at Risk). Drawing on the Discharge to Assess model, SOAR involves coordination among a core team specializing in geriatric care and an interprofessional team over three phases: Prepare, Transition, and Support. Pilot results, reported in the Journal for Healthcare Quality, demonstrate success! On average, SOAR increased the percentage of patients discharged before noon, from 6% to 76%, reduced the time to first home care visit from 49 hours to 6 hours, and cut down hospital length of stay by more than a day. It also led to to improvements with 30-day ED visits and readmissions and facilitated connection to the in-house pharmacy, which allows patients to obtain medications before discharge. Why You Should Come to an ACEs Practice Inquiry Group Have you wondered how to fit screening for ACEs into the care of your patients? Practice Inquiry Groups are explorations of case studies submitted by participants. You should come because it’s a chance to hear first hand how your colleagues in different settings are using #ACEs screening, and a chance to get ideas of how to make your practice more #traumainformed. It's a great chance to connect with family docs like you trying to better care for our most vulnerable patients. In our first Practice Inquiry Group, the person responsible for ACEs screening in a local health system explained why they pulled back from universal screening. We deliberated over the value of universal screening and discussed the on-the-ground practicalities. Who gives the screen? When? To whom? How do we respond to a positive screen?
