Community Care Partners celebrates the 4 esteemed non-profits that comprise our organization: Bay Cove Human Services, Vinfen, Justice Resource Institute, and Mystic Valley Elder Services. Without these partnerships and the dedication of our care teams across all 4 Member Organizations, CCP would not be able to fulfill its mission of improving the health and wellness of MassHealth members across our state.
