About us

Community Care Partners (CCP) is a collaboration of two community-based providers, Bay Cove Human Services and Vinfen Corporation, that offers integrated care coordination to MassHealth enrollees who have complex medical and behavioral health needs. Our multidisciplinary Care Teams partner with enrollees to overcome barriers and meet their health and wellness goals while working collaboratively with healthcare providers and community based social services providers to better meet enrollee needs. Bay Cove Human Services and Vinfen Corporation are leaders in their local communities and fields of service.

  • MassHealth has taken steps toward increasing access to maternal health information and resources by creating 3 webpages for members about doula services, pregnancy, and sexual and reproductive health!

  • Did you know October is LGBTQIA+ History Month? In the context of healthcare, we're encouraged by the progress that has been made over time to advance services such as gender-affirming and HIV/AIDS care. However, stigma and discrimination persist as significant barriers to truly equitable care for LGBTQIA+ folks. Let's take this time to reflect on where we've come from and how much further we need to go to vanquish health disparities for the LGBTQIA community!

  • CCP wishes all of our Hispanic and Latine staff, members and their families a wonderful #HispanicHeritageMonth! National Hispanic Heritage Month runs from September 15th to October 15th and is an observation started in 1968 as Hispanic Heritage Week under President Lyndon Johnson. It was later expanded by President Ronald Reagan to cover a 30-day period. CCP serves a rich, culturally diverse membership with a sizable Hispanic and Latine population. As such, we welcome this time to celebrate the histories, cultures and contributions of our fellow Hispanic and Latine Americans.

  • Massachusetts Advocates for Children (MAC) is a nonprofit children's advocacy organization that works with students, families, communities, and at the policy level to remove barriers to education for children across Massachusetts. To assist families in understanding their children's educational rights, they have designed this wonderful infographic: https://lnkd.in/eXcrQ3Zi MAC also offers a free multilingual helpline that provides guidance, resources, and referrals as needed, to families (including immigrants and migrants) with children who are facing barriers to receiving educational support and services to which they are legally entitled.   • Erin Gadbois, CCP Senior Program Manager and Adaeze Ibe, Bay Cove Assistant Director of Care Coordination attended Brockton's Annual Overdose Vigil this past Thursday in honor of #OverdoseAwarenessMonth. Many organizations such as CCP distributed resources and attendees witnessed a moving program including speakers, singing, and a memorial slideshow of community members who have passed due to substance use disorder.

  • August 17th is designated National Non-Profit Day - a time to spotlight and elevate the invaluable work non-profits do every day to support those in need across all of our communities! Community Care Partners celebrates the 4 esteemed non-profits that comprise our organization: Bay Cove Human Services, Vinfen, Justice Resource Institute, and Mystic Valley Elder Services. Without these partnerships and the dedication of our care teams across all 4 Member Organizations, CCP would not be able to fulfill its mission of improving the health and wellness of MassHealth members across our state.

  • Massachusetts has launched Summer EBT, a new federally-funded program that helps low-income families with school-aged children buy food when schools are closed during summer. Each eligible student receives a one-time payment of $120 on SNAP EBT cards or a Summer EBT card that is mailed out for those who don’t get SNAP. Most students are auto-enrolled and may have received a Summer EBT payment on July 25th. Not auto-enrolled?   • This year, the Brockton Area 2024 Overdose Vigil by Candlelight will be held on Thursday, August 29th at Massasoit Community College in the Quad. The vigil is a deeply moving annual event which brings together people impacted by addiction including families, people in recovery from substance use, and human service workers. CCP will be tabling at the Resource Fair which begins at 6:00PM and a memorial program will begin at 7:00PM. Community members can submit photos of loved ones who have lost their battle with substance use disorder and the photos will be compiled into a video slideshow. Photos can be submitted to the email listed in this post's image up until 8/15.
