More hospitals are lamenting discharge delays that are forcing them to house patients who no longer require acute care. The California Hospital Association has filed a lawsuit over the issue, and the Washington State Hospital Association says discharge delays are adding to financial pressures. Hospital leaders also say the situation is bad for patients.

The Acute Hospital Care at Home (AHCAH) waiver program allows hospitals to deliver acute medical care for Medicare and Medicaid patients at home rather than within their own brick-and-mortar walls. Congress is currently debating whether to extend the program for an additional 5 years. Researchers sought to find out what types of hospitals have adopted the program, as that could inform ongoing policy decisions. Austin Kilaru, Hashem E. Zikry, and David Schriger analyzed hospitals participating in the AHCAH waiver program. In a JAMA research letter, they report that participants have tended to be large, urban, not-for-profit, and academic hospitals. "As further extension of the AHCAH waiver program is debated in Congress, CMS may consider additional strategies to engage a broader set of facilities to develop hospital-at-home programs, particularly nonteaching and rural hospitals," the authors noted.
