Embarking on hospice care starts with understanding eligibility. Here are practical steps to guide you: - Assess the Illness Stage: Consult with healthcare professionals to understand if the illness has reached an advanced stage where hospice is beneficial. - Involve Family in Discussions: Hold family meetings to discuss care preferences and the impact of transitioning to hospice care. - Seek Physician's Certification: A doctor’s evaluation is crucial. They can certify the life expectancy and recommend hospice care based on medical criteria. - Explore Hospice Options: Research hospice providers like Amedicus to find services that align with your needs. - Prepare for Transition: Discuss with the hospice team about how care will be managed, what to expect, and how to address both physical and emotional needs. Hospice eligibility is a journey of understanding and preparation. We're here to guide and support you every step of the way. Many people think hospice care is only for the final days of life, but starting hospice earlier can provide so many benefits. At St. Croix Hospice, we believe that accessing hospice care sooner can greatly enhance the quality of life for patients and their families. Early hospice care means more time to manage symptoms effectively, provide emotional and spiritual support, and create meaningful moments with loved ones. By not waiting until the final days, patients receive comprehensive, personalized care that addresses their needs holistically. Families also benefit from the guidance and support of our dedicated team, easing the burden and stress during a challenging time. Choosing hospice care early allows everyone to focus on what truly matters—cherishing time together and ensuring comfort and dignity for the patient. #HospiceCare #QualityOfLife #PatientSupport #EarlyCare #CompassionateCare #StCroixHospice #DeaconessHospital #LongTermCare

Nearly 30% of hospice patients in 2017 received hospice care for only a week in, and 50% received hospice care for 25 days or less 1. The unfortunate reality is that a few days of hospice care is not enough to provide those who are terminally ill the end of life care they need and deserve. Much of this is due to family members perceiving hospice as a last resort. Choosing hospice doesn’t mean you are giving up on your life or the life of your loved one. Instead, it means choosing a life outside of doctor’s appointments and hospital rooms. Read More: https://lnkd.in/gCTZRXai #EndOfLifeCare #HospiceCare #DignityInCare #CaregiverSupport
