Is Value-Based Care (VBC) the way forward toward transformation in healthcare? While many definitions (and misperceptions) prevail, the featured article notes that "what they all have in common is that we should achieve the best possible outcomes for people receiving care at the lowest cost. It follows, therefore, that value can never be about cost-cutting arbitrarily. Poor outcomes come with a high cost: both human and financial." PROMs (patient-reported outcome measures) are most "valuable as a set of information through which we begin to understand the needs of our patients and understand how we may direct our resources more effectively to meet those needs". Organising Virtual Care: Design, Organisational factors and Health System related aspects, proud on our Mayo Clinics Proceedings, Digital health paper (in press) Abstract: Hospital based digital- and virtual care is becoming increasingly common and its’ reach and scope are expanding in terms of patients groups and technologic sophistication. The objective of this viewpoint is to provide guidance on design and factors that can be decisive for the organization of virtual care from a hospital´s perspective. Relevant aspects to be taken into account are: Characteristics of the technology in a broader sense, the nature and intensity of provider involvement and -supervision, the degree of self-management by the patient and his environment, the relation and cooperation mechanisms with other providers as home care, GP´s and other specialist care, the matter of (economies of) scale and lastly the uniformity of processes over geographic regions and providers. Its a great honour to be invited by the UK Royal Society of Medicine to be their headline speaker at their annual GP and Primary Care conference on 25th September 2024. For those who have been following my work you will know that I have been advocating for the need for a long term vision for health and healthcare in a technology enabled 21st century. A long term vision that can galvanise coordinated and collective action towards a more systemic and holistic model of healthcare that embraces and integrates preventative, early detection and whole person care. The current mechanistic "repair shop" model of healthcare is wholly unsuited to meeting the ongoing needs of an aging population suffering increasing levels of ill health, chronic disease and frailty through loss of function. I will use this opportunity to make the case that short term bootstrapping of primary care - important as it is - is not going to give us the model of continuous, holistic care that is so desperately needed. I will use this opportunity to outline what this future could look like, the new paradigm it calls for and the journey to get there. This will build on my current and previous work e.g: 1. Restructuring the provision of health care around the condition being treated can reduce fragmentation, improve patient care, and lay the groundwork for value-based health care (VBHC). The authors researched the current use of four models in several health care organizations: the Integrated Practice Unit, first described in the context of VBHC in 2013, and three additional condition-based organizational structures defined by the Dutch Linnean Initiative. This qualitative study, using interviews with VBHC experts from 15 large health care providers worldwide, explores these four different ways to implement VBHC. All have different characteristics regarding staff, colocation, level of independence, and leadership. These models have not been previously studied in this way. Patient-reported outcomes (#PROs) are crucial in healthcare research, offering insights into interventions from the patient perspective, and this study focuses on mitigating respondent burden in PROs to enhance data quality and decision-making. After a thorough literature review and a Delphi survey involving multi-stakeholders, the authors compiled 19 recommendations to address this issue including "where possible, consider the use of #ePROs, which may help reduce respondent burden, but must be balanced with the needs and preferences of the target population" and "explore the functionality of ePROs with diverse representatives from the target population where possible". Implementing these recommendations could streamline PRO collection and improve healthcare research and clinical practice. The Canadian Medical Association Journal published a study this month (March 2024), looking at the high rates of physician incivility in the healthcare workplace. More than 75% of health care employees have witnessed uncivil behaviour from physicians,2 and 31% of physicians report receiving weekly or daily rude, dismissive or aggressive communication from other doctors. "Incivility" is a catch all term that could describe actions like: - Interrupting or talking over someone - Sarcastic or demeaning remarks - Ignoring or excluding others - Using dismissive body language - Being rude or abrupt in person or online - many others The study goes on to talk about how toxic workplaces contribute to the problem and the effect that it has on patient care. What it doesn't talk about is how important it is to REPORT that behavior and the barriers to why that does not happen. Whether your a patient or a provider, reporting physician incivility comes at a cost. A lot of those times that cost isn't worth it.
