I’ve noticed that in our field, the opportunities for advancement often feel limited. Many of us find ourselves staying in direct care roles, providing therapy—and that’s it. I know some are content with this path, and I think it also contributes to burnout.
While becoming a supervisor was a cool accomplishment, it didn’t reduce the overwhelming caseloads. Clinic Management or director roles offer a change, but opportunities are scarce and don’t address the diverse aspirations many of us have.
Where are the advancement opportunities for those of us who are thought leaders, creatives, and activists, or for those who simply need a break from direct care? How can we use our unique talents to build community connections, creative marketing, engage in policy work, or sustain our careers if we can’t maintain a caseload of 25+ clients a week? These roles do exist but opportunities are scarce and are often filled outside the therapy world.
Is private practice the answer for all of this? Personally, private practice didn’t provide what I needed to thrive—I crave more daily interaction, collaboration, and support than I could find on my own.

✨“Whatever journey you want to take, I’ll support you through”: a mixed methods evaluation of a peer worker program in the hospital emergency department"✨
📍Results of the study showed that
🔵Support provided to patients primarily consisted of friendly conversations (91.4%),
⚫Discharge planning (59.6%),
🔵Tactics to help the patient navigate their emotions/mental wellbeing (57.8%)
⚫And sharing their lived experience (50.1%).
🔵In over one third (38.9%) of all patient interactions,
⚫Peer Workers shared new information about the patient with the health care team (e.g., obtaining patient identification).
🔵Five major themes emerged from our interviews with Peer Workers which include:
💡(1) Establishing empathy and building trust between the patient and their care team through self-disclosure;
💡(2) Facilitating a person-centered approach to patient care through trauma-informed listening and accessible language;
💡(3) Support for patient preferences on harm reduction;
💡(4) Peer worker role facilitating self-acceptance and self-defined recovery; and
💡(5) Importance of supports and resources to help Peer Workers navigate the emotional intensity of the emergency department.

💡Is conceptualised as ‘an unequal partnership’.
🌿Participants described positive and affirming experiences
💡With individual health professionals and research teams (Respect).
🌿This often occurred within a context of inflexible organisational policies
💡That presented barriers to participation
🌿 Including a lack of financial compensation and expectation to work to inflexible deadlines.
💡As a result, patient advocates could experience a lack of value attributed to
🌿Their experiences and voices (unmet needs from institutions).

To open your own private practice or join a large group practice - it's a big question that many, if not all, therapists will face at some point in their career. We've outlined the pros and cons of each option to help you make the best decision for your career.
Therapists carry the emotional weight of others. This often leads to chronic stress, resulting in burnout and contributing to high turnover in our field.
Over the past month, I’ve attended numerous industry events focusing on improving client care in behavioral health. While I've heard many ideas on enhancing access to care and ways to demonstrate clinical outcomes, few voices have highlighted what evidence shows matters most for therapeutic outcomes: the strength of the relationship between the client and the clinician.
As providers, we understand that our nervous systems need ongoing support and regulation to maintain therapeutic relationships that deliver lasting change. We strive daily to set and maintain boundaries to fully show up for that relationship each and every session.
For employers seeking to recruit and retain clinicians, here are my key recommendations to attract and retain top clinical talent:
🔑 Supervision and Peer Support: Establish a culture prioritizes and encourages regular connections with seasoned supervisors and peers to discuss cases and share experiences. This provides not only professional guidance but also emotional support.
🔑 Professional Development: Offer rich learning opportunities to keep us updated on best practices and new techniques, reducing the stress of feeling unprepared for challenging cases.
🔑 Work-Life Balance: Establish realistic caseload boundaries to ensure we have sufficient time to recharge outside of the room. Provide mental health days and resources for our own support when we, the helpers, need of help.
🔑 Supportive Work Environment: Create accessible and innovative opportunities that rejuvenate our minds, discharge the stress held in our bodies and support our long-term well-being.
By implementing these strategies, companies can not only improve clinician retention and satisfaction but also enhance the quality of care provided to clients, ultimately driving better outcomes and achieving organizational goals.
It’s simple: ask us what we need, listen with the intent understand our work better, and support us in finding ways to manage the stress inherent to our role.
