As I finish my course on Health Systems Strengthening here at the UniversitätsKlinikum Heidelberg, I wanted to share something. Texas had the most hospital closures in the nation and it is becoming a trend in #RepublicanParty states who chose not to implement #MedicaidExpansion. Today's focus for National Health Center Week is: Healthcare for people experiencing homelessness. Did you know Community Health Centers (CHCs) serve over 1.3 million patients who experience homelessness every year? Those facing homelessness are at heightened risk for chronic and acute diseases, behavioral health conditions, and more, which can lead to poor health outcomes, disabilities, and premature death. Our dedicated member CHCs, like Colorado Coalition for the Homeless, are at the forefront, providing essential care, support, and dignity to those in need. Around the world, there are millions of families who live hours, even days, away from the nearest #healthcenter. Being unable to exercise the right to access healthcare leads to deaths or delayed diagnoses for diseases like #malnutrition, whose effects can be irreversible if not treated in time. Community health workers (#CHWs) are key to closing the gap in #healthcare delivery, reaching more people. However, they often operate disconnected from the formal healthcare system, lacking essential medications, necessary equipment and supplies, compromising their well-being, and often, without payment. At #SIC4Change, we have joined the network of allies of the Community Health Impact Coalition, an organization that promotes the design of high-impact community health systems with those responsible for policymaking, financing, training, fostering national #NGO coalitions and CHW associations, and more. While focused on America and a very different developed health system to Samoa, much of what was shared was applicable to Samoas context. Samoa needs primary care at the core of its health system and public health service with public health supporting and, a clear understanding of what 'public health the specialty', 'primary care' and 'public health service' are. We need a robust primary care infrastructure to deliver on the public health concerns for better patient outcomes. We need health workers trained appropriately in primary care and especially when they are posted to rural health facilities with limited resources. Loved the talanoa on value- based payments / contracts #toyinajahi looking forward to the day of primary care services that puts people and patient outcomes first.

Camden Coalition has a bit of a cult-like status within complex care models, and the work there is ground-breaking, mission-oriented and thoughtful. Given my admiration for them, the studies showing their lack of demonstrable results point to a simple fact: Care coordination alone, even when implemented well, is insufficient to reduce hospitalizations for this [really, any] complex population of high-need patients. Primary care has proven its value. I am more concerned that a lot of the innovative, Specialty-focused models seem to rely on navigation, coordination and peer supports - without learning the lessons that are tried and true about complex populations. The reality is that these populations actually need a place to go when their needs escalate, a primary care provider to help them and real specialist access/integration for their needs. Let's hope that the next wave of these innovative companies learn these old lessons - these populations deserve it. https://lnkd.in/ecsjcbY2
