As an EMS medical director, I love the concept of freeze-dried plasma (FDP) as the go-to prehospital resuscitation fluid, with or without blood, (instead of crystalloids) for hypotensive trauma patients. Unfortunately, a review of 607 hypotensive trauma pts from the only 2 RCTs, using both French and German FDP, and 1 pilot study hasn’t found any evidence that FDP improves survival or normalizes INR (marker of coagulopathy) despite study transport times of 37 to 60 minutes. Prehospital ECMO is on the rise. Inner city roadside implementation seems feasible, for more remote or de-centralized areas it should be evaluated if emergency helicopters should offer prehospital ECMO as a standard. Expensive helicopter services equipped with highly trained emergency doctors should add a benefit to ground doctor rapid response cars other than fast transport.
LIFE SAVED: Todd Reuvers has been given a second-chance at life thanks to cutting-edge technology and a team of specialised clinicians. The 53-year-old was placed on pre-hospital ECMO - extracorporeal membrane oxygenation - after having a cardiac arrest at work earlier this year. ECMO provides a patient's blood with oxygen, keeping their brain and other vital organs alive and essentially stopping the clock while the cause of the cardiac arrest can be identified and fixed. NSW Ambulance is investing in excellence in cardiac arrest care with a PRECARE study, which takes ECMO out of the hospital setting and to the patient. In a world-first staffing model, NSW Ambulance critical care paramedics are working alongside specialist pre-hospital critical care doctors in a dedicated vehicle to treat cardiac arrest patients with ECMO. This is a model that has been a long time coming, but there needs to be some changes for it to be fully effective yet. With added responsibility comes added risk, and currently I don’t feel many PTs would accept that added risk without higher compensation. I see this developing through a PMPM model for self insured employers first, and with proper cost saving data progressing into the third part insurers when they agree to reimburse for 97001 at the same rate at they would a PCP evaluation, and increase 97002 to the same amount. The treatment reimbursement will end up being provided by PTAs or athletic trainers until they recognize the reimbursement level for a PT to perform procedures.
