For older adults, maintaining optimal mobility and physical function is a priority. Benefits of a more active lifestyle include reduced risk of mortality and morbidity, fall prevention, enhanced functional capacity, and better managed chronic conditions. The APTA Geriatrics, An Academy of the American Physical Therapy Association recommends older adults have an annual visit with a physical therapist. But, preventative visits in physical therapy are not routine. Lieberz's study will test the feasibility of implementing a preventative physical therapy visit, particularly as older adults start seeing subtle changes in activity. This research will highlight and help categorize the underserved population of older adults with declining mobility. Studying patients who have had preventative physical therapy visits will establish a foundation for future research about the effectiveness of preventative treatment. Most recent article from our team as part of our NIA funded DSM-H R01 study. This is one of the first collecting data directly from racially and ethnically diverse persons living with dementia and their care partners receiving home healthcare. One key here is just how high the symptom burden is in this population...on average 5.4 neuropsychiatric symptoms. A lot of work to do to help this population. If you work for a home health agency and would like to learn how to implement Aliviado Health's QAPI program to improve symptom management and support for persons living with dementia and their care partners, please reach out!!! #homehealthcare #homehealthagency #homehealthcareservices #dementiacare #dementiasupport
We're proud to share the release of our most recent publication in the Journal of the American Geriatrics Society (JAGS)! Led by our fantastic former postdoc Rebecca Lassell who is as of Aug 1 an Assistant Professor at Indiana University School of Public Health-Bloomington. This article, coming from our National Institute on Aging (NIA) funded DSM-H Trial, found that persons living with dementia receiving home healthcare have high neuropsychiatric symptom burden such as agitation or wandering (much more so than in the general population). Therefore home health agencies have the opportunity to significantly improve care and quality of life for this population. If you work in #homehealthcare, reach out to talk to us about implementing our evidence-based quality improvement program to address neuropsychiatric symptoms. Can significantly improve the care you provide driving patient/caregiver satisfaction and referrals!!!

🚑 How Mobile Hospital Teams Are Transforming Patient Care at Home! 🌟 Providing care to patients through mobile hospital teams and home care is an innovative approach that offers a more fulfilling patient experience. 🩺 Patients requiring specialized care, such as those with internal medicine or infectious disease conditions, can receive treatment in their homes instead of being hospitalized, thanks to the use of digital tools and collaboration across different healthcare providers, the primary care system, and the patient's family network. This approach has allowed home care to expand its scope to encompass psychiatry and palliative care, among other fields. 👉 Benefit to Patient includes increased physical activity, enhanced comfort and security in their own homes, and better sleep and appetite. 👉 By delivering care to patients in their own homes, healthcare providers can gain a more comprehensive understanding of their patients' needs and provide a more holistic approach to their care. Get in contact with Skåne Care to know more about how to implement home care in your own context. #patientexperience #homecare #healthcare #prevention #mobilehospital

🎺 You asked, we listened! 🎺 By popular demand, we are now offering SINGLE SESSION registration for our Mastering Dementia program commencing on February 14th. How often as clinicians are we handed (or hand out...) a tool and expected to complete it with no understanding of 'how' to complete it properly, 'why' we need to complete it that way and 'what' it actually means in the first place?! Our first session- Timely Recognition of Cognitive Changes - will walk you through common standardised assessment tools that you have seen and used but may not fully understand. If you are someone who completes screening tools for cognition as a part of your role, then this session with our expert Neuropsychologist Anita Panayiotou is one that you shouldn't miss. This session was run multiple times in 2023 and always receives outstanding feedback. It is suitable for all clinicians working with older people, in the health, aged, community or disability sector. To book your place follow this link: https://lnkd.in/gjfe5pFM We limit our group sizes to allow for an interactive session so there are limited spaces.
