Three medications are now guideline-recommended treatments for heart failure with mildly reduced or preserved ejection fraction (HFmrEF/HFpEF), however, the cost-effectiveness of these agents in combination has yet to be established. For patients with heart failure, MRA was of high value, SGLT2i was of intermediate value, and ARNI was of low value in both HFmrEF and HFpEF subgroups. For patients with HFmrEF/HFpEF increased use of MRA and SGLT2i therapies should be encouraged and be accompanied with efforts to lower the cost of SGLT2i and ARNI therapies

J&J's Tremfya approval in ulcerative colitis yesterday sets up an intriguing competitive case study vs. AbbVie's Skyrizi in UC. While there are nuances to each therapy's mechanism of action, they both fall under the general IL-23 umbrella. Additionally, while no two clinical studies are the same, 45% of patients receiving Tremfya 100 mg SC every eight weeks reached clinical remission at week 44 (see press release), while an identical 45% of patients receiving Skyrizi 180 mg SC every eight weeks achieved clinical remission at week 52 (see Skyrizi HCP website). No notable side effect stands out from the other either. Both therapies provide an important option for UC patients, so it will be interesting to see how they can differentiate from each other. STARgraft is a new AV graft technology designed to provide kidney failure patients with a safer and more reliable way to access the bloodstream for hemodialysis treatment. Its design is aimed at resisting the most common causes of failure in on-market AV grafts: flow blockage and infection. After refinement of the device over a series of clinical trials, STARgraft is showing remarkable promise in addressing these issues and reducing the need for interventions to maintain function. The cohort of twelve patients implanted with the current version of STARgraft has maintained 100% Primary Unassisted Patency (PUP) through 12 months post-implant, meaning zero interventions were required to maintain sufficient blood flow for dialysis therapy. In this nationwide U.S. cohort of patients hospitalized for newly diagnosed HFrEF, >4 of 5 patients were projected as eligible for quadruple therapy at discharge; yet, <1 in 6 were prescribed it. If clinical trial benefits can be fully realized, in-hospital initiation of quadruple medical therapy for newly diagnosed HFrEF would yield large absolute reductions in mortality. It is time to realize the importance of initiating all 4 pillars of GDMT before hospital discharge and without delay. Short- as well as long-term outcomes including mortality are markedly reduced with the early initiation of all 4 classes of GDMT then aggressive uptitration to target/or maximally tolerate dose within no more than 4 weeks (I propose the concept of 4 drugs in 4 days with 4 weeks to target dose).

Most Evidence Should Have an Expiration Date A beta-blocker pill exerts the same pharmacologic effect now as it did in the 1980s. Yet, nearly everything about MI care has changed. In REDUCE-AMI, more than 95% of patients were treated with PCI, dual-antiplatelet therapy, and a statin. Just over 80% received an angiotensin-converting enzyme inhibitor or angiotensin-receptor blocker. Few of these treatments existed in the 1980s.
