I suspended my blog in April and started Project Sword, a data analysis of home health cost reports 2020 - 2022. They present the all-payer margins MedPAC and CMS don't provide. They show an industry in crisis. The new proposed cuts for 2025 will be the end of many agencies, especially small ones, if nothing is done. Stopping the cuts is not enough, HHAs should have all-payer margins similar to other healthcare providers. Here is a comparison of all-payer margins for HHAs calculated in Sword compared to MedPACs analysis of hospitals using all-payers, and the MedPAC recommendations for both in 2025. Great insights into incorporating a collaborative practice analyzing sociotechnical components when conducting Root Cause analysis. We now live and work with humans and machines in healthcare. Ideally we attempt to implement redundancies through the assistance of technology, but alas, errors or rather, serious safety events still occur. When looking at implementing system design to mitigate the “potential” of human error, we also need to look at the inevitable variables that can and will impact the human at the bedside. When compounding factors influence the desired action, the process flow becomes disrupted, thereby opening the door and creating an environment for error/mistake/unintended harm. No one at the bedside starts their day by saying “I am intentionally going to harm someone today”. The term failure is more aptly used to describe the system, and the human is an integral part of the now technologically heavy healthcare system. Safety science should absolutely address the driving forces for human behavior while attempting to use technology as an asset to mitigate future harm.
