Postpartum care is so critical to maternal and neonatal health outcomes. So much education in so short a time. Not only does the new mother have to navigate bleeding, cramping, lacerations/incisions, pain in general, lack of sleep, but also care for a brand new human, breastfeeding, and new family dynamics. Her own safety and mental health can be allll the way down on the bottom of the list. Providing both nurses and new moms with a good foundation for effective and safe postpartum care is essential. Even more important? A place they can go or a number they can call afterward so they can get important education and support--both physically and mentally. One or two postpartum visits 4 or 6 weeks out is not enough.

Let's talk safety in postpartum care- Here are some areas that could use some attention: 👉 Delayed Hemorrhage 🚩 Are postpartum nurses involved in the postpartum hemorrhage drills your hospital is providing? They should be! 🚩 If you have staff floating to your unit to help care for postpartum patients- make sure they are appropriately educated on signs of a postpartum hemorrhage. That isn't always on the radar of a med-surg or float pool nurse. 🚩 Is postpartum hemorrhage risk assessment part of your handoff tool? Make sure postpartum nurses hear about the course of labor so they know the patient is at risk... 2 day pitocin induction, macrosomic baby, chorio, c/section in 2nd stage of labor, etc. are risk factors that need to be passed off in report. 👉Fall risk/ prevention: 🚩 Make sure your magnesium policy addresses fall prevention in the postpartum patient population. 🚩 Magnesium plays an essential role in nerve transmission and neuromuscular conduction. How safe is it for a patient on IV magnesium to be out of bed walking and holding their baby? 🚩 Does your hospital policy address newborn falls? Make sure education is provided (and documented) to parents about the risks of falling asleep while holding the baby in a bed or rocking chair. Purposeful rounding by nurses and aids can help keep babies safe. 👉 Sepsis 🚩 Newborns with early onset GBS sepsis typically start to show symptoms on the postpartum unit or in the newborn nursery. Make sure education is provided to patients and staff for early identification. Sometimes these babies are only seen every few hours by staff and their condition can deteriorate quickly. 🚩 Sepsis is a significant cause of maternal morbidity and mortality. Do you have a sepsis bundle that your postpartum nurses are familiar with? 🚩 Sometimes the anatomical changes of pregnancy make sepsis difficult to detect- make sure all staff is educated on the signs and symptoms of sepsis in the postpartum period. 👉 Failure to rescue 🚩 Once patients get to the postpartum stage of their hospitalization, the frequency of monitoring goes down as the nurse to patient ratio goes up. 🚩 Does your facility do purposeful rounding? Quality improvement doesn't end at delivery. Let's keep our patients safer through the whole hospital stay. What are your thoughts?

Transitional care nurses can lead to better outcomes for older adult emergency department patients. A study published in Academic Emergency Medicine found that transitional nurse care in the emergency department was associated with decreased readmission of older adults during the index ED visit at two of three hospitals, with sustained reduction for the entire 30-day readmission window at one hospital.
