On January 27th, Dr. Jennifer Heidmann (far right) joined Fourth District Supervisor Natalie Arroyo and other local medical leaders for a "community learning session on healthcare provider recruitment and retention in Humboldt County. Over several months, Supervisor Arroyo interviewed more than 40 local doctors, nurse practitioners and medical facility leaders to understand the challenges and opportunities in recruiting and retaining healthcare professionals in Humboldt. She presented her findings to event attendees and facilitated an informative panel discussion where local experts shared their insights, challenges and experiences." Yesterday, I was invited to serve as the guest lecturer for the internal medicine track, presenting to medical residents and interns at Parkland, with the session also broadcast to the VA hospital. My presentation focused on the critical importance of Social Determinants of Health (SDoH) in shaping patient care and outcomes. A new study in JAMA Network Open has found that physicians, nurse practitioners, and physician associates each enhance different aspects of patient care quality at health centers. As health centers strive to make the most of limited budgets while providing the best care possible for underserved populations, the study underscores the value of strategic clinician hiring to address specific health challenges—as well as the essential contributions of differing clinicians as part of transdisciplinary care teams. We're thrilled to highlight this scholarship from study co-author Dr. Benjamin Oldfield, MD, MHS of Fair Haven Community Health Care and Yale School of Medicine, a Fellow of the Association of Clinicians for the Underserved, who joined another ACU member, Logan Stern, DNP, Wilton S., and Dr. Howard Forman for the study. The AAFP urges Congress to vote YES on a legislative package that includes several health provisions that will improve patient outcomes, bolster the family physician workforce, and increase access to primary care. We are excited to see that the proposal includes a 5-year extension of the Teaching Health Center Graduate Medical Education programing, funding for community health centers and National Health Service Corps, reauthorization of SUPPORT and PAHPA Acts, and a partial adjustment to address a forthcoming physician payment cut. I really enjoyed speaking with Stefanie Simmons, MD, because her work with the Dr. Lorna Breen Heroes' Foundation is an example of how advocacy in action can lead to change, even over the course of just a few years. --Dr. Simmons is the Chief Medical Officer of the Dr. Lorna Breen Heroes' Foundation. Over the course of her career, she has served as a clinician, an educator, a healthcare executive, and now Chief Medical Officer. In this conversation, we speak about her advocacy work, and how the foundation has been able to make significant strides in promoting healthcare worker well-being. --In speaking with her, she is clearly energized by her mission: to support healthcare providers in rediscovering the joy in their work, in order to improve the quality of their lives and improve the care that they provide to patients. --She explains that her work is focused on advocacy, awareness, and advancing solutions. As part of her work with the foundation, she meets with state and federal legislators. She also conducts outreach to healthcare organizations . --We discuss the creation of the foundation, which began following the tragic death of Dr. Lorna Breen during the pandemic. One of the contributors to her death was invasive and stigmatizing questions on state licensing applications, such as whether physicians have "ever" been treated for a mental health condition. --The foundation and other organizations have been working to change these questions. Instead of asking about past treatments, the focus should ideally be on current physical or mental impairment. In 2021, there were 17 states that avoided the use of invasive questions about past treatment for mental health conditions. Now, just a few years later, there are 27 states that have implemented this change. --For students interested in advocacy work, or how they can continue to advocate for professional well-being as clinicians, this episode provides some great inspiration as well as actionable takeaways. Those who work in healthcare study and train for years to develop the knowledge, expertise and resulting confidence to make decisions that are in the best interests of their patients. Yet as much as training and knowledge are important in guiding decisions, these alone will not result in excellent care that meets each patient's goals and that aligns with their values. That’s the view of Peter Pronovost MD, PhD, FCCM, UH’s Chief Quality and Clinical Transformation Officer and Veale Distinguished Chair in Leadership and Clinical Transformation. Families, too, he says, have many important things to teach caregivers — consider it their experiential wisdom — especially important in patients with chronic or complex conditions. In fact, Dr. Pronovost argues, the ability to honor this wisdom is essential for safe, effective care. At UH, we’re training all our physicians and staff to be more attuned to important moments with families and to more fully respect the experiential wisdom expressed by the patient's family and loved ones — and to also listen more closely to nurses' and all caregivers' voices.
