When you visit your doctor, do you see the same friendly face each time? Or do you end up repeating your story over and over because appointments are rushed and doctors keep changing? One of the most valuable skills in hospice (and healthcare) is active listening. When spending time with patients and their families, focus on being present to their thoughts, feelings, and stories. Through attentive presence, empathy, and understanding, patients/families are able to express themselves freely and feel genuinely heard. Honestly, this is probably one of the best gifts you could give your loved ones believe it or not, is completing your advanced care plan, i.e. medical living will and/or completing a HCR. A living will addresses health care wishes you would want to or not want to receive including, but not limited to cardio-pulmonary resuscitation, artificial respiration, and/or nutrition or hydration. Other interventions may include pressors, surgical interventions, and/or dialysis. Ultimately, this legal document helps guide your loved ones to make the best health care decisions for your health based on your wishes outlined within that document. A HCR is also known as a healthcare representative. It is someone you elect to make healthcare decisions for you when you are not able to due to an incapacity (you are unable to understand the nature of your disease to make informed decision). Someone who understands your values & health care wishes is the best option to elect as your healthcare representative. These documents both medical living will and healthcare representative (HCR) must be done when you have capacity and must be witnessed. They do not require a lawyer or notary. These documents we are able to complete during an individuals hospitalization, however most times are unable to complete due to incapacity, thus it is important we advocate individuals to complete these forms when they are healthy and can express their wishes at their end of life.
The start of a new year is the perfect time to engage in advance care planning, either completing advance directives for the first time or ensuring existing directives match current wishes for care. Today’s question comes from our E•O•L Deck and asks about how direct you want medical professionals to be. This is typically the first E•O•L Deck question that I ask my hospice patients. In my opinion, this is a very important aspect of quality end-of-life care. Some people prefer to know everything themselves. Others prefer for their family members to be the ones to determine what they know about their disease progression. For individuals that prefer all of the information, I provide them with a copy of Barbara Karnes, RN booklet, Gone From My Sight, the first time that I meet them and we go through it together. We then discuss it again more in depth during my next hospice social work visit, once they’ve had time to read and reflect on their own. The E•O•L Deck is a tool to help professionals and family members facilitate conversations with people approaching end-of-life.
