Did you know that Healthcare in Europe accounts for approximately 5% of the continent´s carbon emissions? Additionally, pollution is linked to approximately one in ten premature deaths in Europe. Prioritizing environmental sustainability within healthcare systems not only benefits the planet but also directly impacts public health. If your healthcare organization disappeared overnight, would it be missed? For hospital-based health systems that provide essential medical care to lower-income patients in urban areas in the United States, the answer is a resounding yes. These organizations not only care for marginalized populations, they also train new generations of physicians, nurses, and allied health professionals. Well done Fiona Garín Mc Donagh for making the link between high value care and environmental sustainability. It is an unfortunate fact that delivering healthcare contributes significantly to GHG emissions and global warming and therefore the illnesses health systems treat. Health systems must minimise their environmental impact through supply chain improvements, energy efficiency etc., but it is equally important to stop low value health activities/interventions; these are not only bad for patients but squander carbon too. Value-Based Health Care should be in everyone's green health strategy and we must be confident that carbon impact created when treating patients is justifed because it is needed to deliver high value care for them. The Biden Administration and the Department of Health and Human Services have issued a call to all healthcare stakeholders to tackle the climate crisis through a new initiative and pledge aimed at reducing emissions across the healthcare sector. The Joint Commission, an accreditor, collaborator and evaluator of healthcare systems, as well as similar accrediting organizations, quickly moved to join what became known as the Health Sector Climate Pledge. They pledged to reevaluate their criteria for hospital systems’ environmental impact evaluations. Evolving state codes, accrediting body evaluations, and movement at the federal level have made it clear: healthcare systems and their stakeholders must commit to aggressive change in their carbon footprint. Some of the most impactful changes they can make are at the facilities level. The challenge is substantial, and for many systems, these waters have gone uncharted. Like anything unknown, myths have built up around what is and isn’t possible. Just published in time for #WorldPatientSafetyDay our paper in Journal of Hospital Medicine on what lessons from patient safety can be useful to prevent harm to the public and the planet. Despite the growing need for healthcare to address planetary health, climate change, and environmental degradation, there are no regulatory and payment levers yet to incentivize action. However, we see opportunities to apply existing patient safety measurement and reporting principles to proactively prepare and accelerate environmental sustainability and climate change initiatives in healthcare. While voluntary programs motivate healthcare organizations to reduce greenhouse gas emissions, metrics are inconsistent and there is no disclosure of verified data, making it challenging to evaluate improvements. Healthcare should apply the principles guiding the patient safety movement to ensure that data transparency and verification are the centerpiece of the transformation to evidenced-based, environmentally sustainable health care.
