Prospective clinical trials ensure that patient recruitment meets specific inclusion/exclusion criteria, multiple specimens can be collected over time (longitudinal), and collection and handling protocols are dictated in advance. Retrospective biobanked specimens for translational studies fail to replicate these rigorous requirements. Do data from clinical trials translate into real world evidence? An overwhelming YES from the ELEVATE pivotal study which once again shows the transformative benefit of chronic LVAD support in Advanced Heart Failure - A median survival in excess of 5 years, a tripling in 6 minute walk distance and near doubling in QOL achieved. Adding life to years and not just years to life. The findings are also echoed by the recent INTERMACS real world analysis (Ann Thorac Surg. 2023 Nov 8:S0003-4975(23)01155-4.). Excited to see the cover story in Vascular News is highlighting our recent open access systematic review and metaanalysis on the potential repurposing of drugs to treat AAA medically by PhD students Joachim Sejr Skovbo Kristensen, Mads Liisberg, Lytfi Krasniqi, Lasse Obel,  Egle Kavaliunaite and I (https://lnkd.in/dDUFsE9t). What a job they did: Of a total of 7,484 screened studies, 39 were included and 12 drug groups underwent meta-analyses. Metformin and statins were statistically significant in slowing AAA growth - even after exclusion of high bias risk studies by -0.73mm/year regarding metformin and -0.84mm/year regarding statins.  However with high levels of heterogeneity. Better studies are needed. Fortunately 4 RCTs on Metformin is upcoming, but RCTs on statins will probably never be performed of ethical reasons. To provide the highest achievable evidence level to convince those not prescribing it to all AAA patients, we need to stick to robust pharmacoepidemiologic studies as we did when we demonstrated that statins reduced the risk of rupture by 25% and 30d postoperative mortalit by 20% ( https://lnkd.in/d3pP-GBd).

🧠 Bioxodes offers new hope for stroke patients. Today we announced we received the green light to continue with our clinical trial with BIOX-101 in intracerebral hemorrhagic stroke (ICH). This follows a positive recommendation by the Data Monitoring Committee, which just concluded a planned review of results from the first 8 patients. “We are highly encouraged by the decision and the fact that there were no safety issues,” said Marc Dechamps, Chief Executive Officer of Bioxodes. The recommendation allows us to enroll the next 24 patients, and we expect an interim data read-out based on the first 16 patients around the end of the year. ICH is a devastating condition, accounting for 40% of all stroke-related deaths. There is currently no approved treatment for these patients. BIOX-101 is derived from a protein found in the saliva of the tick (Ixodes ricinus). It prevents the debilitating secondary effects following within hours after the initial hemorrhagic stroke, such as blood clot formation and acute neuroinflammatory events.
