Principles over protocols... an interactive talk I will give tomorrow to case managers. True personalised care and rehabilitation. Meet the person where they are. Rehabilitation and care must fit to the person and not the person to the rehabilitation. Needs over numbers... There are no set numbers to follow because needs are always changing. Principles over protocols... Teach the principles so the person can decide the next best step. Protocols assume that everyone is the same and must follow the same path. They are not and they do not. Similarly with therapies and techniques. There is no particular therapy or technique. It's the therapist/clinician approach that matters more. The key is understanding the person's needs, the causes of their suffering and together finding the best way forward. Rehabilitation and overcoming chronic pain are both active processes. Both are grounded in learning and growth. We guide and encourage the person to live themselves better -- not wait for something to happen. We help them make it happen in how they live each moment. There is a path for that person. Together we find and follow it until the person can walk themselves. This respects their strengths, values, autonomy and capability. Whole person. Person first. It is now well documented that when “different” bona fide therapies are compared with each other, there is usually no clinically meaningful difference in client outcomes. Whereas therapists’ outcomes do vary significantly. Therapist factors often account for far more variance in outcome than do treatment methods. The Clinical Skills That Matter: Accurate Empathy (Foundational in MI) Positive Regard (Affirmation a key MI Skill) Genuineness/Congruence (Emphasised in MI 4th Edition) Acceptance (Key to the MI Spirit) Focus (A Key MI Process or Task) Hope (Central to MI - Evoke Hope & Support Self-Efficacy) Evocation (A Key MI Process or Task) Offering Information and Advice (Included in MI - Elicit-Provide-Elicit) So, does MI operationalise a helping relationship? Perhaps MI is not meant to be done instead of other treatments. MI is now commonly combined with other treatments . MI is a way of being and doing whatever else you do in a helping context.

Had a tough conversation with a practice owner struggling to keep his head above water His practice looked like it was thriving (from the outside) But ever-higher overheads and ever-lower reimbursements were killing his profitability. A large portion of patients were coming in from referrals. Which kept them "busy" but barely getting by. The problem was his practice had grown to rely exclusively on referrals just to survive, and the referral types were mostly the lowest-reimbursing and/or most time-consuming cases. With margins like this, it almost wasn't worth it. I could tell his heart wasn't in it anymore. Look, I get it Getting new patients through referrals is attractive… it’s certainly a core growth strategy for most practices (and it’s one we help practices build)! But, being fully reliant solely on a referral networking strategy puts your practice in a very precarious and vulnerable position. Build patient lead sources you control.

Join us on 9 & 10 December in the Institute of Tropical Medicine Antwerp for a workshop meeting tackling different blocks of chronic care quality. We will present our conceptualisation of chronic care quality. Researchers will present results of ongoing/just-concluded research from various LMICs in Africa, Asia, and Latin America that look into chronic care quality aims (e.g. access), various determinants (e.g. socio-economic inequities) and indicators (e.g. patient-satisfaction), and the experiences of PAHO in implementing the HEARTS technical package in Latin America. We will discuss about theories of change to improve quality of care, such as policies, leadership and governance; financing mechanisms; and training and education. Interested?
