Great work by the hospital avoidance team at #CALHN AND the community can also play a part in reducing hospital congestion by taking some simple steps for your self and your family/loved ones: 1. Discuss the goals of care with older family members or those with serious health issues - not everyone wants Resus/CPR or ICU 2. Ensure that everyone is aware of what is appropriate/desired and what is not 3. Make sure people's wishes are documented and the document is "findable" 4. Plan ahead - many admissions are the result of people being unwilling to talk about failing health until it becomes critical 5. Organise and accept community services and other help when available EDs and hospitals are very important for acute care - but they are not always the nicest place for those with frailty and gradually failing health. We risk delivering care which is not wanted or helpful when we spend a lot of time and resources without having thought about these simple steps....
Emergency departments are not always the best option for some patients requiring care, particularly for aged care residents or those with restricted mobility and bariatric conditions. To help patients with complex needs avoid unnecessary trips to hospital, the Central Adelaide Local Health Network (CALHN) runs the Hospital Avoidance and Supported Discharge Service, run in partnership with the SA Ambulance Service. The service features a multi-disciplinary team of medical, nursing and allied health care teams who are able to provide face to face and telehealth consultations as well as access to ultrasound, X-rays and a CT scanner. Congratulations to the team working in the Hospital Avoidance and Supported Discharge Service after being named the winner of the Excellence in Strengthening Partnerships category at the 2023 SA Health Awards. Did you know that #urgentcare centers open on Christmas see only 45% of their "regular" volume but the day after Christmas, see 152%? This indicates you'd be better off closing on Christmas Day and doubling up staff the day after. Or that centers open on other major holidays like Thanksgiving and Easter see a sharp decrease in visits, perhaps because patients don't expect the center to be open, while "minor holidays," typically falling on Mondays, see an average >20% increase in visits? We successfully admitted the patient to Meadow Park Rehabilitation & Healthcare Center, where she is received daily inpatient physical therapy for 10 days. The patient was successfully discharged home with #HomeHealth Services by our local LHC Group #HomeCall partner. Today is a good Monday. We have 18 open spots out of 24 for patients to be seen today. As we continue to improve I wanted to share our latest progress here as we work to improve how we provide primary care to our patients. For those of you who have been following along our team has made a significant and concerted effort to improve our access and the experience of care for our patients. We currently serve over 2000 patients and families in our practice and over the last 6 week cycle we have made a concerted effort to make our office more easy to access, a calmer workplace for our employees and a place where we effectively team together. What did we do? - install a doorbell for patient check ins to limit interruptions for our secretary - work as a team to triage all patient concerns rather than delegate this to a single team member - emphasize that prebooked appointments are for care that can only be received in person (well baby visits, chronic disease, antenatal visits) - work to optimize all visits before they are seen in clinic: this means lab work or imaging done before we see patients in some cases to ensure we can optimize our time in person - instill the principle of acknowledging receipt of requests vs feeling pressure to complete them the moment they are received - emphasizing to our team that their time and health is critical to our success and that being calm is more important than having an inbox at zero. - we have opened nearly every day to be fully accessible by online booking and patients are able to book as it works for them. Do I anticipate some hiccups into the future - it's likely.
