Here is my educational moment for today: We all have heard that healthcare differs from everything else. Today, I had a Community Care appointment scheduled by the VA due to ongoing issues with what is believed to be flare-ups from my plantar fasciitis. The doctor walks in and asks me the same questions as usual, then I describe the pain when I first put my feet on the floor in the morning and until everything warms up, burning on the top and bottom of my feet that intensifies throughout the day, hurts in the ankles, etc. Follow-up question, "has anyone ever spoken to you about Tarsal tunnel syndrome?" No, ma'am, it's similar to what people experience with a carpal tunnel just in the ankle but often treated as plantar fascitis. Here is the definition so I don't get it wrong: "Tarsal tunnel syndrome is a compression, or squeezing, on the posterior tibial nerve that produces symptoms anywhere along the path of the nerve running from the inside of the ankle into the foot. Tarsal tunnel syndrome is similar to carpal tunnel syndrome, which occurs in the wrist." She suggested injections in the ankles to see if that would help. I was surprised that after 10 years of treatment and physical therapy, etc., I had never heard about it. She also said she was correct, I would be thanking her in three weeks when we see each other again because the pain would be three times better. The injections in the ankles felt like an electric shock wave because it was on the nerve. One of the secondary diagnoses that can occur after concussion is POTS. It stands for postural orthostatic tachycardiac syndrome. It ultimately means that when we stand up we develop a rapid heart rate (as one of the symptoms). Although we see the heart as beating fast it’s not actually a cardiac issue ~ it’s an issue with our autonomic nervous system. And so, POTS falls under the big umbrella of dysautonomia. Have you ever looked up at the top shelf and started spinning? Maybe in a grocery store or in your own kitchen? I’ve had many patients with top-shelf vertigo. Some even experienced this type of vertigo attack while standing on a step stool, which can be extra scary. The most common root cause of top-shelf vertigo is BPPV. BPPV is an inner ear condition where crystals are loose in your inner ear. When your head is level, the loose crystals are settled. But when you lean your head back to look up, the crystals can slide around in your inner ear and cause a spin. The spin usually lasts for less than a minute, but it can set off a fight-or-flight response which can last for much longer. If you are getting top-shelf vertigo from BPPV, you may also be feeling dizziness or vertigo when you lie down or roll over in bed. Vertigo with looking up, lying down, rolling over, sitting up in bed, or getting out of bed are all common symptoms of BPPV. If you think you might have BPPV, I suggest you consult with a vestibular healthcare provider. The second most common cause of top-shelf vertigo is upper cervical dizziness. Patients with cervical dizziness may complain of chronic neck pain, or they may just have neck stiffness without any pain. If you cannot turn your neck to the right and left 90° without tension, you may have upper cervical dizziness. 🧡👋🏻🎆🎇🧨 On July 5th, 2024, I received life-changing medical care: a DEXA bone density scan of my legs, hips, lumbar (low) spine and neck, that took only a few short, painless minutes to complete. 💥💥 The reason for referral, in this scenario, was that recent x-rays (pictured above) of my spine and ribs revealed I had been living with two fractured ribs since February of 2024, despite visiting emergency rooms and urgent cares in my geographical area 5➕ times to report my pain and refill my daily prescription medications while living without access to a primary care provider. 🏥💊 The results of referral in this scenario turned out to be positive - but just in the sense that we found indication of disease. (I personally am not pleased; I am grieving.) 🚨🌸🌸🌸🩻🩻🩻🌸🌸🌸🩻🩻🩻🌸🌸🌸🚨 As of July 5th, 2024, I now know I have statistically low bone density in my hips, my lumbar spine and my neck. My rib fractures in early 2024 were due not to any physical trauma or injury but rather due to a slow, internal collapse of the very structure of my body. My skeleton has been diagnosed with ‘OSTEOPENIA’, the precursor to ‘osteoporosis’ that tends to come for adults above the age of 50 who have lived a full life of labor and experiences. Headaches are common after concussions. A post-traumatic headache (PTH) is considered persistent if it continues more than 3 months after a TBI. A 2017 study found that 51 percent of people with a TBI experience headaches within the following 2 weeks. After 3 months, 23 percent had persistent headaches. Scientists are still researching whether post-traumatic migraine headaches might benefit from distinct treatment plans. For now, post-concussive migraine headaches are treated using the same methods as migraine attacks that are not caused by concussions. Limiting your physical activity and getting rest are the most common recommendations from doctors. Bed rest is sometimes recommended, but not as often. If you believe you’ve sustained a concussion, seek immediate medical care. After receiving care for a TBI, if your headaches become more frequent or intense, or don’t go away on their own, speak with a doctor right away.
