It's having a transformational impact on value-based healthcare in Australia. “The Risk of Hospitalisation Module uses an algorithm developed by CSIRO based on primary care data. This is potentially a game changer in the way we do things in general practice because this tool allows us to calculate what the risk is for each patient in our practice ending up in hospital in the next 12 months. This sort of insight allows us to look at things differently. We can see where the care gaps are, we can see where the needs are in our patient population, and we can tailor the care according to those needs. Check the program https://lnkd.in/dzDZzK3M “Value-Based Health Care (VBHC) and Person-Centred Health Care (PeCHC) are important approaches to health care which aim to improve the outcomes that matter to people. VBHC seeks to do this by organising care around patients with a specific condition or common needs, standardising outcome measurement (using clinical and patient-reported outcomes) and cost measurement, using this data to perform comparisons and learning from those with better outcomes and the same or lower costs. PeCHC takes a more individualistic view that relates to the context, goals and preferences of a single person. PeCHC also enables meaningful involvement of people receiving care in health service design and health system strengthening. When considering the implementation of both of these approaches to health care delivery, we realise how little we know about the preferences and goals of people receiving care, and the extent to which we have met them and achieved the desired outcome.
