As a junior doctor in a public hospital, I often grapple with the blurred line between good medical care and service excellence amid resource constraints and unrealistically high expectations among some patients and family members. For example, an angry patient demanding for an MRI to be done the very same day in the absence of any medical urgency. This initiative sparks contemplation on the delicate balance between public and private healthcare. How can a not-for-profit model navigate service expectations while staying true to its mission? These questions resonate with my experiences, prompting valuable reflections on the evolving healthcare landscape. Benefits include: Patient Preference: Many patients prefer receiving care at home due to the comfort and familiarity of their environment, which can positively impact their recovery experience. Safety and Confidence: The majority of surveyed individuals feel safe managing medications and care at home, indicating trust in home-based healthcare services. Faster Recovery: A significant portion of respondents believe that recovery is quicker at home, potentially reducing the overall burden on healthcare facilities. Cost Efficiency: Hospital-at-home care can lower costs by reducing the need for hospital stays, which often include higher overhead and resource expenses. The latest elective waiting time statistics are out from the Department of Health - Ireland, and they tell a fascinating story. The numbers are jaw-dropping. Ireland has 54,000 outpatients waiting longer than 18 months, compared with about 2,000 in England. And looking at waits for inpatient and daycase treatment, Ireland has 7,000 waiting over 18 months compared with 3,000 in England. Now recall that Ireland has a population of about 5 million, compared with about 57 million in England, and we can see that this is a long-waiting backlog on a vast scale. And yes, they are doing something about it. Ireland has a commendably detailed Waiting List Action Plan (WLAP). With any elective waiting list, the main challenge is to treat/remove patients from the waiting list faster than they are added to it, and the WLAP demonstrates an excellent grasp of this, with a clear distinction between the recurring and once-off elements. Their focus on referral and pathway management processes is also welcome and important, as these unexciting but important processes are key to sustainable improvement. (NHS England, take note.) However the analysis struggles when it moves from the waiting list to waiting times, placing too much reliance on over-simplified metrics which do not sufficiently increase understanding or support good management of the waiting time problem. In one case – the publication of average waiting times – there is a risk of misleading the public with numbers that do not represent the waiting times they can expect in real life. As the Irish Department of Health point out, countries all over the world are struggling with long waiting times following the pandemic. As a relatively small country, Ireland is well used to looking beyond its borders for experience and solutions, and probably better than England at doing so. Both the good and not-so-good parts of their plan reflect the international level of understanding of this complex subject. So if the Department of Health of Ireland – or indeed of any other state experiencing these issues - would like to expand their perspective and understanding still further, they are very welcome to get in touch. A recent study has confirmed the potential of home telemedicine-based hospital care. Patients who received hospital-at-home (HAH) care experienced shorter hospital stays—two days less—without an increase in readmissions compared to those in traditional in-hospital care. Most strikingly, HAH care was actually linked to a significantly lower 30-day mortality risk, with a 66% reduction for COVID-19 patients. Medibank is calling for a significant expansion of hospital care in the home across Australia to alleviate pressure on the healthcare system. A new KPMG Australia report commissioned by Medibank reveals the incredible potential of hospital care in the home, highlighting: Cost savings: Up to $1 billion in healthcare costs and $6.4 billion in capital expenditure by 2030 Improved access: Around 350,000 patients could receive hospital-level care in their home in 2030 Enhanced care: Hospital care in the home offers quality care comparable to traditional hospitals, with lower risks of hospital-acquired complications and high patient satisfaction Hospital care in the home replicates the level of care provided in traditional ‘bricks and mortar’ hospitals, providing acute care to patients in the comfort of their own homes using a combination of technological interventions and direct face-to-face care by doctors, nurses and allied health professionals. Medibank CEO David Koczkar welcomed the report, which responds to calls from patients for more ‘in-home’ care. “Australia is currently over-reliant on traditional inpatient care settings at the expense of access and choice for patients. There is a health transition underway and we are trailing behind many other countries when it comes to the uptake of more fit-for-purpose and economically sustainable models of care,” Mr Koczkar said. “Bricks and mortar hospitals will always play an important role in the health system, but expensive acute hospitals should not be the only option. We know patients want the option to be treated in the comfort of their own home and it frees up precious beds for those that really need it.
