As part of our commitment to helping all adults in Santa Clara County navigate their health and life options, we offer several different care management programs depending on the needs of each client. MSSP works with frail older adults 65 years of age and older who are at risk of nursing home placement and would like to remain living at home. Our care managers work with the client, their loved ones, and medical professionals to develop and implement an individualized care plan. As a Family Nurse Practitioner (FNP) working in Federally Qualified Health Centers (FQHCs), I’ve devoted my career to serving underserved Latinx communities. Despite my cultural and linguistic proficiency, I often find myself overlooked in favor of providers who lack the necessary connection to these populations. This imbalance is particularly frustrating as I work toward certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP), driven by the urgent need for mental health services in underserved areas. While some employers claim to support healthcare equity, I’ve encountered resistance to my requests for flexible scheduling, such as 10- or 12-hour shifts. These accommodations would allow me to balance my current work with advancing my skills to better serve the community. It’s disheartening to see other providers granted this flexibility while my requests are denied, raising questions about the priorities within these organizations. One clinic has offered me an opportunity, for which I’m grateful, despite the daily 30-45 minute commute. I remain committed to serving the Latinx population, even when faced with challenges that include balancing work, school, and personal responsibilities. However, this situation reflects a larger issue in healthcare—providers with essential cultural competencies are too often undervalued. Healthcare organizations need to prioritize those who are best equipped to serve vulnerable populations. Being in the industry for years now, I know the staffing mandate is not the answer and one we all are fighting against. What I would be curious about is how many agency staff in this study were perdiem vs. true contract travelers that nursing homes were/are using. I believe we need to control the script, recruit and retain and move forward out of chaos. I have a few ideas. 💡
