In 2020, my oldest daughter Gabby was complaining about pain in her feet. I initially chalked it up to growing pains but after meeting with 2 doctors, we got the diagnosis: tarsal coalition—a rare condition in which the bones in BOTH of her feet were fused. We were told surgery was necessary, but after asking the surgeon in CT just 2 questions, he seemed annoyed as if I was questioning his expertise. Being so close to NYC, I sought out a second opinion at the Hospital for Special Surgery. HSS is ranked #1 in the world for orthopedics. Patients know *what* HSS does and *how* they do it, but it's the WHY that people seek out their services and keep coming back: "To provide the highest quality patient care, improve mobility and enhance the quality of life for all, and to advance the science of orthopedic surgery, rheumatology and their related disciplines through research and education." For our family, that meant a pediatric orthopedic surgeon who knew what tarsal coalition was and had done the surgery several times. She also took the time to listen to our questions and ran through the pros, cons, and ins and outs of everything. We knew exactly what to expect. In May 2021 and January 2022, Gabby's surgeries were a success. Today, she can run races, dance, and play soccer—all without pain. She also continues to see the same doctor at HSS for other issues that were discovered later. I'm thrilled to announce that our publication, "Racial Disparity in Postoperative Complications Following Shoulder Arthroplasty (SA): A Systematic Review and Meta-Analysis," has been accepted by the Sage Journal "Shoulder and Elbow." Congratulations to all the co-authors for this outstanding achievement, especially Ramish Sumbal for his exceptional guidance and mentorship. Key Highlights of Our Study: ♦️Our analysis included 14 studies, encompassing a total of 1,781,783 patients. ♦️Black Patients had higher risk of post-SA complications compared to White patients. Odds Ratio (OR): 1.32 (95% CI 1.25–1.39; p < 0.00001; I² = 0%). ♦️White Patients: Increased risk for readmission. ♦️Hispanic Patients: No significant overall risk compared to White patients. This publication marks a significant step in addressing racial disparities in orthopedic surgery and highlights the importance of equitable healthcare outcomes. It's been over a decade since Best Practice Guidelines (BPGs) were published to decrease surgical site infection (SSI) in pediatric spinal deformity. Unfortunately, successful implementation hasn't yet been established. Columbia Orthopedics faculty Michael Vitale and Benjamin Roye MD-MPH contributed to this study evaluating surgeon compliance the BPG. The hypothesis was BPG authors and surgeons with more experience, higher caseload, and awareness of the BPG would have higher compliance. This morning, A female patient arrived for a pre-THR surgery consultation. She weighed 295 pounds. She was 58 years of age and was 5'6'' tall. Her BMI was off the scale; the examination confirmed she needed a THR. What to do... I knew that my anaesthetist college would not be that excited about going ahead with the procedure, with all the extended potential risks and possible complications. She needed her to lose weight; any amount would be advantageous, but ideally, it would be beneficial to see a reduction of at least 50 pounds. We know that anything that involves exercise would not be suitable as the pain in the hip is extreme. She was directed, with a bit of persuasion, to download and complete the Oxford Therapeutics psychology-focused weight loss programme. Are you encountering an evergrowing surge in obese patients?
