When I read that an organisation is launching an investigation post incident, I worry that they are going into it with a mindset of rooting out wrongdoing. Who in their right mind would ever voluntarily participate in an investigation where you’re expected to incriminate yourself or your close friends and colleagues which could result in you being disciplined, dismissed or civilly liable? We need a change of mindset. Instead of investigation why not call it a review where we identify the LESSONS IDENTIFIED (the areas for improvement) but also the NOTABLE PRACTICES (the good pieces of work that should be replicated again). We take those things and crate Specific, Measurable, Achievable, Realistic, Timely (SMART) recommendations which crates a real and honest culture of learning and development to turn those LESSONS IDENTIFIED into LESSONS LEARNED. This is the carrot to improve processes, not the stick to scapegoat an individual. #JOL | #OrganisationalDevelopment | #LessonsLearned
