We advocate because the system we’re living in is broken, and it’s time for change. We’re pushing against the barriers set by Fresenius Medical Care and DaVita Kidney Care because they created a world where patients are just numbers, where the technology is outdated, and where the truth about what’s really happening is hidden under layers of corporate rhetoric. Advocacy isn’t just a word; it’s a call to action, a lifeline for those who have been silenced by a system that was supposed to protect them. A stands for **Accountability**. We demand it from the corporations that profit from outdated technology, technology that’s supposed to save lives but instead risks them with unreported adverse events. Patients deserve better, and it’s time we hold these organizations accountable for the harm they cause. D stands for **Demanding Change**. It’s not enough to ask nicely. We’ve been polite long enough, but the system hasn’t changed. We demand a shift in how patients are treated, how technology is developed and implemented, and how the voices of those most affected are heard. V stands for **Voices**—yours, mine, everyone’s. We need more people speaking out, sharing their stories, pushing back against the status quo. We can’t do this alone; it takes a collective effort to break down these walls. O stands for **Outreach**. We have to reach out to each other, to teach, coach, and lead. This isn’t just about shouting into the void; it’s about connecting, building a community of advocates who are united in purpose. C stands for **Change**—systematic, deep, meaningful change. We can’t settle for surface-level fixes that allow these organizations to continue with business as usual. The change we seek is fundamental, affecting every level of care, from the technology used to the policies enacted. A stands for **Awareness**. We need to raise awareness about the outdated and inferior technology being pushed onto patients both in centers and at home. These machines are old, unreliable, and often unsafe, but the organizations responsible are too slow to respond, more concerned with profits than patient safety. C stands for **Care**—true care, the kind that listens, understands, and acts in the best interests of the patient. It’s what’s missing in this industry, and it’s what we’re fighting to restore. Y- stands for You because without you—your voice, your action, your willingness to spread the word—we can’t make the difference that’s needed. We’re standing up for those who are too weak, too sick, too weathered to fight. But we need your help. These large dialysis talk about improvements, but what we see are machines that should’ve been retired years ago, technology that’s failing those who need it most. And when things go wrong, they sweep it under the rug, ignoring the adverse events that should be addressed immediately. The FDA inquiries? They drag their feet, stalling, delaying, doing whatever they can to avoid the costs of real change #dialysis

In healthcare technology innovation, we often trip somewhere between noble intentions and actually improving how humans deliver and receive healthcare. We bias toward simplicity and quick execution then fail at delivering usable help to the people in the healthcare system – providers, patients, caregivers… the humans in healthcare. But there are glimmers! There are people with big brains and bigger hearts like Ruben Amarasingham, MD, a brilliant and caring physician with a calling: to improve healthcare delivery for real people. These guys head right at complexity, then with care, apply their powerful brains and tools to solve real problems (see screenshot below). - Standards are amazing - USB, SQL, REST, JSON and so many more. - Standards are ok but slow, eg FIHR. - Standards are friction creating when scoped wrong- eg cloud computing portability. Advice: create balance across vendors and pick the right level. Eg objective is acceleration of smart hospital; but start with practical items within it that are actionable vs top down vague concepts. Better standards will help more innovative vendors and hospitals firsr and that is a great objective to pull the industry towards positive innovation faster !
