Medication reconciliation is a simple process that can be performed by healthcare providers. The process typically involves the following steps: 1. Gather the patient's current medications. This can be done by asking the patient to list their medications, reviewing their medication list, or reviewing their pharmacy records. 2. Gather the patient's prescribed medications. This can be done by reviewing the patient's medication order sheet or by talking to the prescriber. 3. Compare the two lists of medications. This involves looking for any discrepancies between the patient's current medications and their prescribed medications. 4. Resolve any discrepancies.
