[PCNL/ECIRS - Position 5 - Prone split leg position] After knowing the different positioning, can we actually perform prone ECIRS? Yes, indeed!   Back in 1993, Dr Michael Grasso (together with Dr Richard Nord and Dr Demetrius Bagley) described the prone split leg position for simultaneous antegrade and retrograde access to upper urinary tract. They also described the difficulty in performing ECIRS - two skilled endoscopists, dual light sources, various lithotrites, and several rigid and flexible endoscopes - that echoes the issues that hinder the development of ECIRS. Clinician Review of the AdaptivEndo Ureteroscope System The AdaptivEndo team recently conducted clinician interviews regarding the AdaptivEndo ureteroscope system in preparation for upcoming Design Verification and Validation (DV&V) activities. Dr. Ryan Malone, Dr. Krishnanath (Krish) Gaitonde, and Leveda O'Bannon, RN evaluated the AdaptivEndo ureteroscope and controller during a series of hands-on labs utilizing benchtop anatomical models. [PCNL/ECIRS - Position 3 - Lateral position (1)] Apart from supine and prone, PCNL has been done in lateral position. Dr Kurt Kerbl reported a PCNL in flank position for a morbidly obese patient in 1994 [1]. Lateral position is also suitable for patients with deformities like kyphosis or lower limb contracture [2]. ⚠ Case of the week! There has been a lot of criticism against vaginal meshes (and with good reason). In this small clip, you see how an anterior mesh may hold the bladder back (no cystocele). However, this patient eventually came to see us, because she still has to deal with a serious enterocele and a small rectocele.
