Linkage of health workforce datasets to patient datasets in hospitals and registries and primary care should be a priority

Is your local general practice on your street? In your suburb? Can you get there easily? For many Australians, the answer is no. While options like telehealth can be helpful in some situations for some people, in-person care will always be needed. Even where services could be virtual, the default is often that you need to travel to receive care. And where there is neither a service nor a health professional nearby, the safety and quality of your healthcare may well be reduced. Today, I’m in Canberra for a collaborative meeting about medical workforce planning. Whether you are a health leader or a healthcare user, you already know that: 🩺 The health workforce is not evenly and fairly distributed across the nation: your postcode determines the local health care available to you. 🩺 Australia’s ageing population, and likely increase in chronic disease, means that the demand for healthcare will increase. 🩺 Urgent improvements in recruitment, support and retention of our health workforce are needed. Affording essential healthcare is tough right now. For those living outside urban areas, even having the money won’t guarantee access to a local service. While we work this through and implement the effective solutions to these issues, a strong parallel focus on health prevention is needed. Every Australian should have the resources to keep themselves well. They should also have timely access to healthcare when they need it. For folks living in rural and remote Australia, and other places where workforce is thin on the ground, skills in, and knowledge of, health prevention is crucial to leading healthy lives. In an ideal world, we would layer workforce planning over rigorous population health needs analysis. This would ensure that access to care as an essential first principle in shaping health workforce planning. We are not there yet but it is where we need to go next. The collaborative spirit in the room today gave me hope. By bringing together diverse perspectives and expertise, we can unpack these challenges. Using both the wisdom and good will in the sector shared solutions can be developed and put in place. How would I increase #access to #generalpractitioners? This is the thought equivalent of 'napkin math' but I think it's directionally accurate: 1. Our population is aging 2. We have a relatively fixed amount of medical graduates 3. We do not want to go backwards on the amount of qualifications needed to perform a task (we are generally a more risk averse society and risk is moderated by knowledge and experience) 4. We have multiple studies showing GP led primary care is a net positive for efficient and excellent care for the Australian population (Starfield etc) 5. Therefore we should leverage our medical workforce to provide more care/value per doc 6. This means increasing funding to general practice (increased Medicare rebates and block funding and registrar training pay) 7. This will increase numbers of med grads going into GP 8. Then have headline billing of Medicare (nurses and allied health services attract Medicare rebates for patient care using supervising GP provider number) allowing and promoting GP led multi D team care. Straight talk from former SE Health CEO, Shirlee Sharkey, on tackling the persistent challenge of hallway healthcare. In this 2019 Longwoods Publishing essay, she argues there are three main ways to address it: ➡️ Shorten hospital stays by discharging people faster; ➡️ Avoid hospital deconditioning, especially among frail older adults; and ➡️ The BIG opportunity – keep people out of hospital in the first place. Five years later, with the growth of integrated and transitional care partnerships, health systems across Canada are starting to make progress on the first two -- but there is still much work ahead to build robust community-based systems of care that will unlock the big opportunity.
