The Program of All-Inclusive Care for the Elderly (PACE) is our best model to address siloed healthcare, keep people healthy (not sick), and help people live longer and better lives. I am a big fan of value-based care in theory, but the devil is often in the details. PACE is a model that actually works. It improves outcomes, reduces costs, and wraps into one the best of Medicare, Medicare Advantage, and Medicaid Long-term services and supports. We need nursing homes to stay open. We need assisted living and personal care communities to stay open. But many are closing their doors. Every older adult needing nursing home care has a legal right to protect assets and pay for that care with Medicaid. But for every resident on Medicaid, the nursing homes lose money every day. The state sets a Medicaid rate to pay nursing homes, but the rate does not keep up with inflation. As the ranks of older adults swell, care facilities are under pressure.
