"Virtually there or still more to do?" Good progress has been made over the last couple of years to scale up virtual wards in the midst of a range of competing operational priorities, with 9,700 virtual ward beds now in place ahead of winter. But there are still multiple barriers preventing further and faster implementation from a recent HSJ article: 1. The right number and mix of staff Effective virtual wards requires the right mix of staff with the right skills. Senior, experienced frontline staff, including consultants and senior nurses, are needed, while clinical leadership is essential for effective decision-making and risk management. 2. Sustainable funding from integrated care systems From 2024, the expectation is that virtual wards will be built into plans as business as usual. But in the face of tight budgets and a demanding and complex delivery task for ICSs, there are concerns that funding for virtual wards could be squeezed. 3. Buy in from all staff and patients The latest figures from July show that the occupancy rate of virtual wards beds stood at 64 per cent; below NHSE’s target of 80 per cent by September. While a lot of work to support this is taking place at a local level, there is an important role for national leaders and politicians to play in raising the profile of virtual wards and amplifying these messages. Over the past 7 days one of my family members has spent over 24 hours (over 2 sessions) in the WVT Hereford A&E Department. That is after waiting 12 hours for an ambulance. They were suffering with severe chest pain and breathing difficulties. Now Hereford A&E, is it any wonder that staff retention is at an all time low when they are subjected to such appalling working conditions? For patients however, it is an absolute disgrace, the waiting area for the whole of A&E, where my family member spent most time is effectively a small corridor. A small corridor with very limited seating. An area where on Thursday evening, there were as many people standing as were seated and on Monday up to 8 over 70's were left standing. I can only think that this is part of the ex Prime Ministers attempt to reduce the aging population. Nobody should be subjected to this appalling environment. There is clearly not enough space in this "new" waiting area for A&E and combined with the lack of space where triage is attempted, patient care and dignity is severely compromised. Combine this with a GP system which attempts make access to a doctor ever more difficult you have a structure which is not fit for purpose across Herefordshire.

What is a freestanding emergency room? Texas is one of a small number of states that allows privately owned/operated emergency centers. This means that many Texans have an option to avoid the hospital ER waiting room prior to receiving medical care. You can think of a freestanding ER as providing a level of service in between that of urgent care clinics and hospital ERs. 🔶Urgent care: Typically no doctor on site, open during business hours only, and no lab or CT scanning. 🔶Freestanding ER: Doctor on site 24/7, lab facilities and CT scanning on site.
