Last week, whilst attending a course on pelvic pain and neurodynamics hosted by Sandy Hilton hosted by Pain Geeks in the Netherlands, I encountered a unique challenge. Dealing with respiratory issues during the sessions, I found myself taking frequent breaks to manage my symptoms. It wasn't easy, but it highlighted the essential role of pacing in self-care. Living with Long Covid presents its own set of obstacles. However, seizing opportunities doesn't mean pushing through at all costs. This time, I chose to attend the course while prioritising my well-being. Stepping out of the room to have an episode of coughing then resting my head down for a few minutes every hour. Pacing myself allowed me to participate while respecting my limitations. As clinicians, it's crucial to remember that self-care isn't a luxury—it's a necessity. So, how do you ensure you're taking care of yourself when you're feeling under the weather? And do you pace your day even when well? Dr Amy-Jo Farrow is an Occupational Health Physician in the U.K., and a burnout coach, helping exhausted doctors thrive. Olivia Morris is the Co-Founder and CEO of Verity Barrington and Doctors Living in Ireland, having been a physician advocate for the past 10 years. We discuss how doctors know about healthy living and what to recommend to their patients, yet they frequently do not take care of themselves in the same way. We dive into some reasons behind that, and how it's almost seen as a "badge of honor" not to take care of one's basic needs during a work shift. That is not sustainable and should not be the culture! We talk about the need to help doctors discover solutions that can be adapted for each physician's unique life. We each share a vulnerable story of how as a physician, a physician's wife, and a healthcare professional, in a time of crisis, how the notion of "the patient always come first," no matter the personal cost, took its toll. Doctors having an identity outside of their primary employment is so important, and can include building a portfolio career, exploring hobbies...whatever brings you joy and gets you energized about life! Message to doctors: Physicians need to practice radical self-compassion. Know that there are options out there, and people who can help you. Never feel like it's just you, it is not, and there is so much support available! Message to patients: If you come across a "grumpy" doctor they may be having a bad day, or missing an important family milestone to be there to care for you. Be compassionate towards another human being, it can make all the difference. Doctors AND patients need to take responsibility to restore the human connection in healthcare! In medicine, where the goal is to help others, we often forget to apply the same level of care to ourselves. That's why understanding the importance of compassionate boundaries is key. Realising that we have our own limits is crucial. Constantly working without adequate breaks doesn't make us more effective, it can lead to burnout. Setting compassionate boundaries means establishing limits that safeguard our well-being—it's not selfish, it's necessary. By clearly defining these boundaries, prioritising tasks, and delegating when needed, we ensure we stay fit to provide the best possible care. Compassionate boundaries aren't just about saying no, they're about saying "yes" to our well-being, allowing us to keep serving from abundance. I'm still recovering from my last endo operation. The pain is endless, the journey is deeply problematic, and the research still has a long way to go. It's heartbreaking that on top of having this condition, you also have to be an advocate for yourself every step of the way. Fighting through being belittled, feeling unheard, and disregarded in the health system. It's exhausting. I can't express how much having a supportive place to work in makes a difference. It's why I choose to freelance so I can be adaptive, but this also comes with its own huge burdens of not having the structures in place to support you when you need to be in hospital, or crawled up in a ball. I really want to push for more support in this area. I am gearing up to start a new collaborative creative project exploring this, so would love to connect/chat more with anyone that's interested. If you don't know much about endometriosis, please do look into it and consider how your work practices can be more adaptive and empathetic to those experiencing chronic pain.
