Pediatric orthopedics is a branch of children’s medicine that treats bones, muscles and joints. An orthopedic pediatrician can perform surgery when necessary, but they also perform other treatments, such as braces or casts. An orthopedic pediatrician is involved in treating young children to teenagers when they experience pain, discomfort or abnormalities in their bones, muscles or joints. . 😲Did you know that children with Cerebral Palsy should have an annual spine X-Ray to monitor for scoliosis? 😬Have you not done an annual spine X-Ray for your child with Cerebral palsy because you think that they don't need it? 🧒Children with cerebral palsy are at a higher risk of developing scoliosis, with scoliosis occurring at younger ages in individuals classified at a higher GMFCS level. In addition, the incidence of scoliosis continues to increase up to the age of 20–25 years. As such, routine Spine Surveillance is essential for early detection and timely intervention to prevent or manage the progression of spinal curvature. 💪 The Spine surveillance process typically involves the following components: 1. Regular Clinical Assessments: 👩⚕️Paediatricians, Orthopaedic specialists, or Physiotherapists, perform regular clinical examinations to evaluate the alignment of the spine, look for signs of asymmetry, and assess muscle tone and strength, which can impact spinal stability. 2. X-rays and Radiological Evaluation: 🩻X-rays are often used to obtain a clearer picture of the spine's alignment and curvature. The frequency of X-rays will depend on the severity of the cerebral palsy, the presence of scoliosis or other spinal issues, and the Orthopaedic specialist's recommendations. 3. Implementing Preventive Measures: 🧑⚕️Paediatric Physiotherapists will recommend various strategies like dynamic garment orthosis, strengthening exercises, or rigid bracing to help stabilise the spine and prevent scoliosis from worsening. 4. A very sad day… as I read in The Sunday Times newspaper from the UK that Dr Yaser Jabber, a pediatric orthopedic surgeon at the world renowned Great Ormond Street Hospital for Children NHS Foundation Trust, has potentially harmed 731 children. As per the news article ‘…GOSH has begun an urgent review of 721 children seen by Jabbar in its orthopaedic department. This review is being conducted by five expert surgeons and is expected to take another 18 months to complete. Of the 37 cases that have been reviewed already, 22 children have been found to have come to some degree of harm, 13 of them classed as “severe harm” — that is, likely to include lifelong injuries.’ Understandably, serious questions are being asked about the culture, management and approach to safety at GOSH, renowned around the world as one of the best children’s hospitals. The review of Dr. Jabbar’s care at GOSH follows a confidential investigation by The Royal College of Surgeons of England which compiled a damning 100-page report on Jabbar’s practice and the wider department. It was completed in October last year but not made public. The report is highly critical of the wider culture of GOSH, as well as what it labels the “dysfunctional” department where Jabbar worked. It warned the lower limb reconstruction service was not “safe for patients or adequate to meet demand”. And it goes on… a number of staff at GOSH said they raised their fears in writing. “We were asking for meetings with managers but they never responded. It’s frustrating, you feel helpless,” one said. Jabbar’s behaviour was finally exposed after a fellow surgeon, Sarah McMahon, who joined the trust in 2020, blew the whistle. McMahon formally raised concerns about Jabbar in June 2022, when she wrote to senior managers. This led to a meeting with the trust’s chief medical officer, Sanjiv Sharma who commissioned the Royal College of Surgeons report. Why? Why? Why? Just last week I was with a group of healthcare leaders in the US describing the UK’s Bristol Enquiry in the late 1990s on poor outcomes of pediatric cardiac surgery in Bristol. 30 years later… we have more data but not greater accountability. This is just the tip of a very large iceberg. How long before we have an objective, trusted and data-driven approach to patients and their families being able to identify the best surgeon, cardiologist, neurologist or gynecologist for their care? https://lnkd.in/eqFSHXmT
