I am reposting this SPSP — Society of Product Safety Professionals instructor profile of Donald Mays because this "Fundamentals of Product Safety" program in September is going to be an excellent overall learning opportunity for attendees. In particular, Don is a great speaker with wide-ranging experiences from which we can all learn--especially, me! He is a regular guest lecturer in my product-safety engineering (and ethics) course in the University of Utah Dept. of Mechanical Engineering every Fall. But, Don is only ONE of the numerous outstanding instructors who will be there to speak and to answer questions. To paraphrase Otto von Bismarck, "it's unnecessary to learn from your own mistakes when you can learn from mistakes made by others." DAY 3- HAVE YOU HEARD OF THE 4D'S? If you haven't, you are missing out! Kicking off the operational learning questions with these bad boys- initially there was the three Ds (Dumb, Dangerous and Different) - courtesy of the awesome Ivan Pupulidy, PhD. Now thanks to my great friends Brent Sutton, Brent Robinson and Jeffery Lyth, we know them as the four D's with the addition of the term 'Difficult'. So what are they? How do you use them? Ask those at the pointy end of the stick questions similar to these: 1. "Tell me something we make you do that is DUMB?" (And if you are offended by the word 'dumb', then switch it out with the words, "what doesn't make sense"? 2. "What about this task makes it DANGEROUS?" "Is there anything DIFFERENT from when you last did the job to today?" "Show me what makes this task DIFFICULT for you?" The answers to these questions will provide you with insights that will help you with as Dave Provan says, "the safety of work" instead of just "safety work."

This year, Black & McDonald (B&M) celebrates 10 years of producing the B&M Safety Calendar. Submissions were requested from kids aged 4 to 12 years and was open to all B&M and subsidiary employees. Typically, 2000 copies are printed and distributed to all offices across B&M and subsidiaries. There are 13 prizes for the featured images and 6 prizes for honourable mentions. All submissions have a designated space in the calendar. The premise of the calendar is to get younger generations to start thinking and talking about safety! Submissions touched on many different safety topics such as school yard safety, games or sports safety, weather safety, or drawings that show their parent or family member donning the appropriate Personal Protective Equipment (PPE) at work. Regardless of the drawing, the common feedback we received was that when kids are engaged, it creates an opportunity to open a dialogue about important safety topics… and they get to see their artwork featured in print! CBO recently achieved a Total Recordable Incident Rate (TRIR) of 0.00 in 2023! This impressive metric shows consistency in our employees making every effort to be incident free. At our work sites we hear and see mottos like “Never Pass a Fault”, “Live and Work Safely”, “Home Safely Always” and “Nobody Gets Hurt Today or Tomorrow” – these values are never far from anyone’s mind, demonstrating the effectiveness of our safety program. Today, as we commemorate the World Day for Safety and Health at Work, we reflect on our commitment to safety at every level of our organization. At Quaker Houghton, safety is our guiding principle, captured in our core value to Live Safe. Throughout the past week, our annual Safety Week unfolded across our labs, customer and processing locations, offices, and manufacturing facilities. With a focus on hazard identification and situational awareness, our teams engaged in discussions, trainings and other activities aimed at empowering every individual to recognize and mitigate risks effectively. As we continue on our path to Zero Incidents and Accidents, our mindset is clear: stay vigilant, always improve, speak up when it matters, and be dedicated to a culture where safety is non-negotiable.

This was my second of three presentations this week. This one was for the Board-Certified Safety Professional's Virtual Global Learning Summit. My session was entitled "How to plug the cheese: barrier thinking at the front line." This session introduced concepts of barrier thinking with a focus on making it relevant to front line employees. We defined what barrier thinking is and why organizations need this type of approach. I provided an overview of layers of protection analysis and bowtie methodology. In turn, those concepts were explored from the perspective of the front-line worker. The concepts came to life through several creative ways of engaging front-line workers. We also explored ways to make sure barriers are in place before work begins. Combined, these methodologies reduce the likelihood of significant injuries and fatality potential events. The graphic describes a case study I presented on a gasoline fire that was started by static electricity. It's a cool way to explain some core concepts of barrier thinking. BTW.. plugging the cheese refers to eliminating the holes in layers of protection (layers of Swiss Cheese) that allows accidents to happen. SafetyAnd Consulting Associates, Inc.
