Every day, we work tirelessly to address the health-related social needs of individuals on behalf of our health plan partners, addressing gaps, improving outcomes, and making a difference. Take a moment to hear the story of Lisa, a member we recently helped after her situation prevented her from getting the care she desperately needed. It reveals why and how our approach of bringing local knowledge and local resources together works — on the ground, where it matters most. https://lnkd.in/gq6T4xWH

Welcome back to #UnpackedwithPapa, where we unpack the belief that social health is health 🌟 In this episode, we’re joined by architect and design researcher Erin K. Peavey, AIA, to learn just how big of a role the built environment plays in our social health and well-being. Erin speaks to The Foundation for Social Connection ’s new SOCIAL Framework report, which explores how the built environment can be both a driver of and a barrier to social connection, as well as guidelines and strategies for how we—as individuals, as communities, as healthcare institutions—can take action and influence our built environment to foster more connection, rather than discourage it. 🌟 Watch the full episode: https://bit.ly/4boagdO 📄 Dive deeper into the report: https://bit.ly/3QOoblf 🖥️ Sign up for the Foundation for Social Connection’s “Advancing Social Connection through the Built Environment” webinar on May 21st: https://bit.ly/4bFnvXi #BuiltEnvironment #SocialConnection

Leading a Needs Assessment with UNV: A Public Health Thrill Ride! For public health enthusiasts, there's nothing quite like diving deep into the heart of a community's health needs. That's exactly what I experienced leading a needs assessment proposal during my time with the United Nations Volunteers (UNV) program. Going beyond the surface, we meticulously collected data – surveys, interviews, focus groups – to understand the intricate challenges and aspirations of the community. But this wasn't just about identifying problems. We were determined to co-create solutions with the community itself. Collaboration was key. Fostering meaningful dialogue, building trust, and empowering local stakeholders became our guiding principles. We weren't just there to assess; we were there to empower. Of course, late nights fueled by caffeine 😅 were par for the course as we navigated complex data, strategized, and refined our proposal. But every hurdle was met with resilience, knowing the impact could be life-changing. Now, as the proposal nears completion, anticipation thrums through me. The possibility of tangible improvements in communities worldwide is both humbling and invigorating. So, to my fellow public health warriors, let's keep pushing boundaries! Together, through collaboration, community engagement, and collective action, we can pave the way for a healthier, more equitable future 😊 . Here's to the power of #UNV in shaping a better world! #PublicHealth #CommunityEngagement #GlobalImpact #PositiveChange #StayTuned

Camden Coalition Board of Trustees National Member; Retired OTR/L; Former DEI Chair for CHES Foundation; Global Patient Advocate; Keynote Speaker;
Attention: Pittsburgh Steelers, CVS Health, CVS Pharmacy - PA folks get ready for one of the best conferences on the planet to take place in your city! If you and your organization have not registered you are going to want to be at this one; do so immediately- availability remains. Unlike other conferences, I have attended as a Global Patient Advocate-( abroad and stateside) this event brings innovative yet practical, thoughtful and compassionate leader servers in the healthcare community together sharing how we do the work we do (nationwide and internationally) with those we are blessed to partner with daily. At this event, every person will be inspired to be the difference we all want to see in the world with real-world practices and strategies on “how to”accomplish the goal of building lasting partnerships. Also, I am looking forward to presenting with Risa Tolbert, RT(T) in the Collaborate and Partner with ALL of ME, Please! session. Healthcare equity and authentic, community engagement practices will be shared throughout the keynote, panel discussions, beehive presentations and workshops. Most importantly, you can sign up for on-site visitations-to see where the service and work is being implemented locally while at the conference! Of all the conferences, I attend or participate in annually, it is my #1 in all metrics: quality of content shared; diversity (DEI) in participation in all aspects; real-world recommendations that can be successfully implemented across organizations/hospitals/clinics and EVERYONE is (seen and heard) when sharing input and insights. See you all there!
We are officially two weeks away from connecting with everyone in Pittsburgh! Every community has an ecosystem of care. How can we be intentional about making those ecosystems coordinated, person-centered, and equitable? This work goes beyond short-term partnerships. Join us in Pittsburgh this October to learn from folks across the country and across sectors who have been tackling this exact issue. Putting Care at the Center is the annual conference that brings together the national complex care community to learn, network, and create a shared agenda to improve care for people with complex needs. Explore this year's agenda, workshop session descriptions, site visits, and more at https://lnkd.in/eN34YcmE
