Historical Cardiology Milestones (Part 5): Heart-Lung Machine Invention (1953) In 1953, Dr. John Gibbon (1903-1973) developed the first successful heart-lung machine, which allowed surgeons to perform open-heart surgeries by taking over the function of the heart and lungs during the procedure. In 1930, after witnessing a patient’s death during a pulmonary embolectomy, Dr. John Gibbon conceived the idea of a machine to support cardiac and respiratory functions during surgery. Over the next decade, he and his wife Mary developed devices that maintained complete pulmonary cardiac bypass in cats for 25 minutes. In the late 1940s, Gibbon convinced IBM President Thomas J. Watson to help create a more advanced device. And the rest is history. Cardiology, the study of the heart and its functions, has a rich history with numerous milestones that have significantly advanced our understanding and treatment of heart disease. An innovative cardiac catheterization technique was performed at Hillel Yaffe Medical Center! This technique is designed to address severe calcific narrowings in the coronary arteries. This allows for the opening of specific arteries through shaving and crushing the atheromatous plaque in the inner walls of the artery using a diamond-coated crown. Dr. Rami Abu Fanne, interim head of the catheterization unit, who performed the procedure together with Prof. Aharon Frimerman and Dr. Yaniv Levi, notes: “This is an extremely innovative technique intended for complex cardiac catheterizations for patients that suffer from specific lesions that cannot be opened/passed using conventional techniques.” A drill-like device, Diamondback 360™ Orbital Atherectomy System by ABBOTT, was used. We treat the artery with a rapid rotational movement while modifying the atheromatous plaque and crushing the calcium layer of the inner arterial wall. This allows us later to expand the narrowed segment with a balloon and implant a stent in an optimal way. The significant advantage of this method is that bypass surgery may be avoided. The technique replaces other less controlled drilling techniques with the risks they entail. The Top-Ranked Heart Hospitals in the Great Plains Region: Among the top 50 rated U.S. hospitals for adult cardiology care in 23-24, four were based in the seven-state Great Plains region. This included the Mayo Clinic, the 3rd highest-rated heart hospital in the country. Prolonged VA #ECMO after cardiac surgery, PELS-1 cohort study, investigating outcomes 🔍2K adults included from 34 centers/16 countries, 2000-2020 ⏱️support duration 0-3 days = 32.1%, 4-7 = 38.3%, 8-10 = 13%, >10 = 16.5%; nearly 30% of patients supported for > 7 days ⏳long-term post-discharge prognosis comparable to PC ECMO patients with shorter run ↪️ U-shape curve of mortality & #ECLS duration observed in this cohort: lowest morality if run duration of 4–7 days, higher if > 10 days; longer run (> 7d) also associated with complications like bleeding, AKI, #sepsis, MOF, but postdischarge survival good. Decision to continue vs cease PC ECMO complex: critical reevaluation should be performed after ± 7 days, and multidisciplinary teams should take age, type of surgery, comorbidities, complications (all more often described in nonsurvivors), and patient values into account. The TAVR explant pandemic is upon us! (Credit: Shinichi Fukuhara) Just last week we removed a TAVR that migrated into the LVOT, requiring an MVR. What was once an isolated AVR, became a TAVR explant and double valve. TAVR explant is not a straightforward case, the morbidity and mortality are high. Habib Jabagi MD, BSC, MSc, MLP, FRCSC, cardiovascular surgeon, recently spoke at The Houston Aortic Symposium “Frontiers in Cardiovascular Diseases.” This conference invites cardiac surgeons, cardiologists, vascular surgeons, and other healthcare professionals involved in the care and treatment of patients with aortic diseases to discuss diagnosis and treatment paradigms for aortic diseases. Dr. Jabagi presented a case report titled, “Challenging Complications in TEVAR: A Case Report of Contained Aortic Rupture, Intraluminal Graft Thrombus, and Endoleaks,” discussing the complications that may result from thoracic endovascular aortic repair (TEVAR), a minimally invasive procedure used to repair a thoracic aortic aneurysm or dissection, and how to properly address them, which sometimes require complex redo open repairs like this case. #Cardiology #CardiacResearch #CardiacSurgery #MedicalResearch

The European Society of Cardiology echoes the classification of acute #myocardialinfarction based on tissue injury severity proposed by the Canadian Cardiovascular Society some months ago. I am intrigued by this innovative development in the field of Cardiology! A recent study suggests that using ultrasound as an adjuct could potentially replace the traditional method of resynchronization therapy with 3 pacemaker wires. This advancement raises the question of whether single lead pacemakers could be substituted with this groundbreaking technology.
