Knowing that outcomes will not improve without addressing social needs of patients and their families, innovators are looking at the viability of ACOs focused on social needs that place primarily non-clinical staff at the center of care. These ACOs for Medicaid managed care move away from clinical staff screening for social needs, but instead rely on enablement organizations skilled at coordinating social services to drive care and pulling in the clinical team as needed.
