The NATO Centre of Excellence for Military Medicine conducted a workshop on ‘Observations and Clinical Experiences: Ukraine 2022-present,’ delivering a wealth of insights that challenged our perspectives, particularly in planning for large scale combat operations. Key insights included: 1. The imperative of scene safety, with medics facing the threat of drone targeting. 2. The complex dynamics of ‘whole society war,’ necessitating the treatment of both military personnel and civilians. 3. Addressing challenges such as prolonged evacuation times through considerations of triage, whole blood transfusions, and the appropriate use of pain medications and antibiotics. 4. The critical role of standardization in saving lives, underscored by initiatives like Tactical Combat Casualty Care (TCCC), robust data collection methods, and the implementation of clinical governance protocols. It is exactly 277 years since the British Navy surgeon James Lind wanted to treat scurvy, a lethal disease at the time, and tested different potential cures on 12 sailors. ⛵ He allocated the men into six groups and treated them with different medications. Lind evaluated the results of different groups, following the principle of comparing like to like: the treatment results were the best with the group that was given oranges and lemons. 🍋 Lind's groundbreaking work is considered the first clinical trial, which is why 20 May is celebrated as Clinical Trials Day. Treatment and evacuation in the austere environments. Over the past 20 years, the need for urgent resuscitation has become a more prominent focal point for mobile medical teams. The evolution of dynamic and complex operations has substantiated the need for unconventional extraction, evacuation, and casualty care. During the latter months of 2020, an expeditionary resuscitative surgical team performed and extended evacuation of a casualty utilizing resources such as alternate means of transport and a contracted personnel recovery team. The event was reviewed by a research team to highlight their capabilities, their considerations during the operation, and the care provided to the casualty. They published their findings along with some considerations for the future of austere casualty care and evacuation in the 2021 Winter edition in the Journal of Special Operation Medicine. One of the best blood bank teams in MD
We’re excited to kick off ‘Blood on Board,’ a public-private partnership between the Maryland State Police, Maryland Institute for Emergency Medical Services Systems (MIEMSS), and our blood bank at the University of Maryland Medical Center. Whole blood, used for emergency transfusions, is now available on MSP Trooper helicopters for prehospital air medical response for critically injured patients. Maryland is the first state in the country to offer this lifesaving Blood on Board initiative as a statewide service—a better state of care.
