How does Care Management differ from Patient Advocates and Case Managers? Here's a quick overview: ◾ Care Managers - Usually work in private practice helping individuals and families through holistic assessments - Create plans of care - Make recommendations/link to community resources & ongoing support. ◾ Patient Advocates - Tend to be more medically based. - Work with many individuals who have acute/chronic health conditions. - Help individuals achieve optimal health and well-being. - Work with client’s medical bills and advocate for cost effective service. - Review medical records and are good with navigating the medical field. ◾ Case Managers - Typically hospital or physician practice based. - Work in the medical field and are paid by insurance - Help coordinate care, transitions, discharge planning, reduce hospitalizations. - Can be in-person or telephonic. A main difference between care managers and case managers? Case managers provide a more specific service to get a specific outcome. An example is a case manager leading the discharge planning in a hospital who does specific education on discharge and follow up. This means that case management services typically end within a time frame; such as discharge from a hospital.
