This week is Dementia Action Week (16 to 22 September). Dementia is Australia’s second leading cause of death, affecting approximately 411,100 people in 2023, with numbers expected to double by 2058. In the Brisbane South PHN area alone, an estimated 15,445 people live with #dementia. The theme of this year's Dementia Action Week is ‘act now for a dementia-friendly future’. One small way carers and health workers are helping create a dementia-friendly future is through the free My Health Journal resource. The My Health Journal is a free, user-friendly tool that helps people living with dementia to manage and track their healthcare appointments, GP communication and other important health information all in one place. Journals are available in English or simplified Chinese. Learn more and order My Health Journals here 👉 https://loom.ly/0UurdH8 #dementia #dementiadiagnosis #DementiaActionWeek #BrisbaneSouthPHN

Today is World Sepsis Day — a day for raising awareness about the life-threatening condition that arises when the body’s response to an infection damages its own tissues and organs. Sepsis is known as the ‘silent killer’ as its symptoms often mimic that of other, less serious illnesses such as a cold or gastroenteritis. That's why it's important that people know the signs of sepsis and their healthcare professionals always ask, "Could it be sepsis?" Worried about a patient? Trust your gut and escalate. 👉 See the adult sepsis referral pathways: https://lnkd.in/gVTBHMMG 👉 Download Children’s Health Queensland's paediatric sepsis checklist for your patients: https://lnkd.in/g8m66_id #sepsis #SepsisAwarenessMonth #WorldSepsisDay #healthcareprofessionals #BrisbaneSouthPHN #QueenslandHealth
